    Observation: Water from the handwashing sink at KITCHEN was measured at a temperature less than 100°F. 92F
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
  • Hand Drying Provision / Individual, Disposable Towels
    Observation: Observed that fixed paper towels were not provided at each handwashing sink. Hand towel dispenser missing from wall.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. AS DISCUSSED, ROLL OF PAPER TOWELS MUST BE IN A HOLDER, TO NOT BE TOUCHED OR BE LIKELY TO FALL ON FLOOR BETWEEN USES (OR) YOU CAN SIMPLY USE, A STACKABLE PILE OF PAPER NAPKINS (STORED AS FAR AWAY FROM SINK AS PRACTICAL).

SANITIZER: Establishment has 2-Vat, not 3-Vat sink. Discussed wash/rinse/sanitize process with person in charge. Did not have Chlorine test strips, but had mixed to an acceptable level > 50 and < 200 ppm. EHS provided small supply of test strips and copy of color key.
CERTIFIED FOOD MANAGER AND ORS PHOTO ID REQUIREMENT: The previous director and CFM is no longer present. The new director, Sedinam Bruce, was not present, but other workers indicate she will be taking the CFM test. EHS explained requirements to Betty McManus and to church administrator Chris Howard, and provided ORS/CFM information sheet to both persons. CORRECTIVE ACTION REQUIRED: WITHIN 10 (TEN) CALENDAR DAYS (BY OCTOBER 26, 2014), PROVIDE DOCUMENTATION OF CFM CERTIFICATION AND ORS CARD TO EHS BY EMAIL: john.vandervoort@fairfaxcounty.gov, OR BY FAX: 703-653-9448, Attn: John Vander Voort, RE: Merrifield Child Development Center. FAILURE TO PROVIDE SUCH DOCUMENTATION IN A TIMELY MANNER MAY LEAD TO FURTHER ENFORCEMENT ACTION. NOTE: Food service is minimal. The establishment serves only snacks and breakfast, not involving cooking, and minimal food prep, using disposable plastic and paper ware and disposable gloves (witnessed by EHS)

  • Critical: Certified Food Manager/Presence Required
    Observation: There is no Certified Food Manager present in the establishment.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.   • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Milk at 53F in True 2DR cooler.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CORRECTIVE ACTION: Milk was discarded.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action. CORRECTIVE ACTION: FAX LPROMETRIC CERTIFICATE AND CERTIFIED FOOD MANAGER ID TO: 703-385-9568, ATTN: John Vander Voort, RE: Merrifield Child Development within 14 calendar days (by June 3, 2013)
  • Controlling Pests by Eliminating Harborage Conditions (repeated violation)
    Observation: Harborage conditions exist.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises.
  • Removing Dead or Trapped Birds, Insects, Rodents, etc. (repeated violation)
    Observation: An accumulation of fresh mouse fecal matter found in floor level cabinets on both sides of the 2-compartment sink.
    Correction: Remove dead rodents, birds, and insects from the control devices at a frequency necessary to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests. CORRECTIVE ACTION: REMOVE FECAL MATTER, EMPTY ALL KITCHEN CABINETS, CLEAN ALL SURFACES THOROUGHLY, AND INSPECT FOR INGRESS OPENINGS.

The purpose of today's visit is to conduct a follow-up to the routine inspection conducted on November 19, 2012. The facility received a pest control treatment on November 28, 2012. Majority of the cabinets has been clean, however there is accumulation amount of mouse droppings in the cabinets near the stove that needs to be clean. EHS recommends another pest control treatment this month.
  • Removing Dead or Trapped Birds, Insects, Rodents, etc. (repeated violation)
    Observation: Dead insects and mouse droppings were found on the premises.---Observed mouse dropings in the cabinets near the stove.
    Correction: Remove dead rodents, birds, and insects from the premises to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.
