    Observation: The handwashing facility located at the by 3 comp. sink is blocked, preventing access by employees for easy handwashing.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use. Cleared.
  • Critical: Plumbing / Repaired Per Law
    Observation: A recent repair to plumbing system components of the hand sink by 3 comp. sink not operating are not in accordance with law.
    Correction: A plumbing system shall be repaired according to Law. Fax the required repair documents, with-in 10 days, to our office at phone # 703-653 9448 (PLEASE PUT INSPECTOR NAME, YOUR NAME AND YOUR FOOD ESTABLISHMENT NAME, ON FAX. THANK YOU.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Critical: Storage of Toxics, Not Above Food, Linens, Equip., Single-Svc. (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.--OBSERVED SPRAY BOTTLE OF MULTI-SURFACE CLEANER ON CLEAN STORAGE RACK ABOVE CLEAN UTENSILS.
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination.--CERTIFIED FOOD MANAGER (CFM) REMOVED BOTTLE AND STORED PROPERLY.

  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink on the cook-line being used to dump ice and fill up containers.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. Discussed with manager at time of inspection. Discussed with food employees at the time of inspection.
01/21/2014Routine
Purpose of this inspection was in response to a call from the Fire Department reporting odor and noxious gas in the restaurant. HAZMAT team was called in but could not identify the cause of the odor/gas. HAZMAT team ruled out either a gas leak or sewer leak but the Fire department suspected a freon leak but could not pin point exact location. According to the Fire Department one person an employee reported getting sick and was rushed to hospital. Restaurant had closed voluntarily as a precaution. EHS arrived at the scene and traced smell to the back of the ice cream making machine (possible compressor leak). EHS requested person in charge to open drive through window and main door to allow fresh air in. After about 40 minutes the smell disappeared and while still typing report, the employee who had been taken to hospital reported back to work saying she had been advised that she was fine. Ice cream making machine has been unplugged from power, advised PIC to take it outside and reopen restaurant.
  • Equipment / Good Repair / Operation (corrected on site)
    Observation: Odor/noxious gas coming from ice cream making machine.
    Correction: Turn off machine and do not use the unit till repaired and operating properly. Fax work order to 703-385-9568. Machine has been turned off and odor/noxious gas stopped coming from machine

09/05/2013Risk Factor
The purpose of today's visit was to conduct a risk factor assessment inspection in conjunction with a complaint investigation. A complaint was received by the Health Department on September 3, 2013. The complainant visited this facility around 12:30 am on September 1, 2013. The complainant alleges that a smoothie was ordered and that an object that looked like rodent feces was found in the drink. EHS spoke to the district manager on the facility phone during the inspection. The District Manager was aware of the complaint and shared that she had spoken to the complainant on the phone today and resolved the customer's concern. The shift manager brought the EHS the alleged drink that was returned by the complainant. EHS observed a half full mango smoothie with multiple small brown particles floating throughout the smoothie. On the inside bottom of the smoothie drink, EHS observed three larger size brown particles that appeared to be parts of chocolate chips. If the drink observed during today's inspection is in fact the same drink that the complainant returned the brown particles appear to be pieces of chocolate chips. The same blender is used for fruit smoothies and McCafe drinks. EHS observed multiple chocolate chips in the drain of the blender spray washer. Manager shared that the drain is cleaned at least once a day and the blender is changed and/or washed, rinsed and sanitized every four hours. EHS observed contracted pest control reports. Pest control is received at least once every month. Complaint cannot be confirmed.

The purpose of today’s visit was to conduct a routine inspection. Renovations completed October 2012. Several certified food managers on staff. EHS provided manager with updated Employee Health handouts in English and Spanish for future use with any new employees.
MAINTENANCE:
*Water Heater: Bradford White EF100T199E3NA which uses 199,999 BTU with posted recovery rate of 238.8 GPH

  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site) (repeated violation)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Handwashing Signage at Handwashing Facilities Required (corrected on site) (repeated violation)
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. EHS PROVIDED EXTRA SIGNS TO MANAGER.
