Observation: The floor drain in the rear food prep room was observed in a state of disrepair.
Correction: Replace the floor drain cover to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the floor drain, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: Floor wall juncture in dry food storage rom and throughout the kitchen is not coved and closed to no larger than 1/32 inch space.

Observation: Harborage conditions exist. Approximately 65 live fruit flies observed resting on the walls, and flying throughout the facility.
Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Clear floor drains of old, odorous decaying food in food prep room.

Dishmachine was under routine maintenance during inspection. Record shown that rinse temperature was measured at up to 200 F on the thermometer. Person in charge is advised to adjust boost heater temperature to maintain final rinse temperature between 180 - 194F to provide proper sanitization of the utensils. Overall conditional of the facility is sanitary during inspection.

Observation: Toilet room door is not tight fitting. and/or provided with a self- closing door
Correction: Provide toilet room door that is tight fitting. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
