Notes: Greasy sludge and food debris under ice machine - manager thinks it is from overflow of the grease trap at the three part sink. The grease trap needs to be cleaned or serviced so it does not overflow. Floor needs to be cleaned under ice machine. Missing ceiling tiles in places - exposed insulation hanging down from ceiling - repair. Good handwashing observed.

Other observations: Floor drain by ice machine has blockage. Sewage backing up onto floor. PIC turned off ice machine and called service person who is scheduled to arrive here as soon as finished with last stop. Floor was mopped. Thoroughly disinfect floor. (Service man arrived). Interior of warming cabinet is soiled. Clean. Change wiping cloth solution. Clan walk-in floor under mat. Broken hood filter. Repair. Surface of counter beside drive thru is in poor repair (wood splinters are evident). Surface to be smooth and cleanable. Mildew along wall behind 3 compartment. Fan grill in walk-in is dirty. Clean. Cooked onions/peppers had a 8 hour time placed on it. I asked for clarification. If this is a TCS food, four hours is maximum.
