Observed no certificate available for establishment certified food manager and employee on shift at time of inspection unable to reach manager by phone. Employee stated that Manager Lindsay Meeker listed on previous inspection is no longer employed at this location. This violation must be addressed by Monday, August 11, 2014. **Warning**

  • Basic - Bathroom facility in disrepair. In women's restroom, observed tank lid missing from toilet with a piece of cut cardboard covering toilet tank. Also observed handle on toilet tank broken exposing a sharp edge.
  • Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container. Observed open can of red bull on back area prep table. **Corrected On-Site**
  • Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed open can of red bull on food prep table in back area. **Corrected On-Site**
  • Basic - Soap/paper towels/hand antiseptic provided at a non-handwash sink. Observed soap and paper towel dispenser over prep sink in back area. Observed precooked chicken strips thawing correctly in prep sink and when asked manager on staff what kind of sink it was her reply was prep sink. Establishment was advised to remove handwash aids at prep sink to prevent possible future violations of hand washing at a non handwash sink.
  • High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands.
  • High Priority - Employee washed hands with cold water. Observed employees wash hands at handwash sink at front counter where hot water is not working/not supplied.
  • Intermediate - Hot water at handwash sink does not reach 100 degrees Fahrenheit. Observed max temperature reached at handwash sink at front counter was 70°F.
  • Intermediate - Hot water not provided/shut off at employee handwash sink. Observed at handwash sink at front counter. Manager was asked about sink and she advised faucet was not in correct position for hot water. When faucet was repositioned per her orders, the water cut off all together.
