- Observed gaskets/seals on cold holding unit in poor repair. Cooler with glass door.
- Critical. Observed accumulation of debris in warewashing machine and associated equipment. Lime scale buildup inside dish machine.
- Equipment compartment not equipped to properly drain accumulation of moisture. Water leaking from lift top cooler into pan on floor.
- Critical. Vacuum breaker missing at hose bibb. Outside back door.
- Critical. Outer openings of establishment cannot be properly sealed when not in operation. Gap at bottom of back door large enough for insects, rodents to enter.
- Floors not maintained smooth and durable. Cracked floor tiles with pieces missing.
- Critical. Observed a splitter/multi-plug adapter in use. For reporting purposes only.
