  The establishment does not have an employee health policy for the prevention of foodborne illnesses. I prvovided the person in charge with a copy of the DC Food Code employee medical referral, interview, and reporting agreement forms.

  The establishment does not have an employee health policy regarding the prevention of foodborne illnesses.
• Food-contact surfaces: cleaned & sanitized
  Food debris is on the bottom shelf of the reach-in freezer. Onion skins and other food debris is on the floor of the walk-in refrigerrator. There is an accumulation of dust on the walk-in refrigerator's compressor fan guard.
• Proper cold holding temperatures
  None of the food items held in the reach-in refrigerator were held at the proper temperatures. All items measured at least 46 degrees Fahrenheit.
• Consumer advisory provided for raw or undercooked foods
  Kitfo (Ethiopian style steak tartare) is listed on the menu and there is no consumer advisory notice regarding the health risks associated with the consumption of raw or undercooked animal foods.
• Proper cooling methods used; adequate equipment for temperature control
  Neither the walk-in refrigerator nor the reach-in refrigerator are maintaining the proper temperatures. All items measured in the reach-in refrigerator were measured at temperatures above 41 degrees Fahrenheit.
• Food properly labeled; original container
  The rolling bins of dried food items are unlabeled. (Corrected On Site)

  The injera bread does not have any nutritional labeling and there are two (2) different brands sold in the establishment.

  There are loose kitchen ceiling tiles and a missing ceiling tile above the mop sink. There is an opening in the ceiling above the fire suppression system. A cutout (large opening) exists in the wall adjacent to the garbage disposal unit. Also there is a gap under the front door; I was able to slide a folded in half 32-page small newspaper/pamphlet under the door.

  A bag of onions is stored on the floor in the walk-in refrigerator. (Corrected On Site)

  Plates and bowls are stored on a rack in the kitchen with the food-contact surfaces unprotected and these items need to be invoerted or covered. (Corrected On Site)

  Aluminum foil is lining two (2) of the shelves at the front counter. (Corrected On Site)

  There are open seams and cracks in the wooden cutting boards. One of the wooden cutting boards has a gash on the cutting surface. The plastic cutting boards are not smooth and have numerous grooves.

  There are no chemical test strips available to measure the concentration of sanitizing solutions.
• Nonfood-contact surfaces clean
  There is a heavy accumulation of grease on the ventilation hood filters and sprinklers. The exterior of the ventilation hood has an accumulation of grease. There is a heavy layer of grease accumulation on the fire suppression system tank/canister.
• Physical facilities: installed, maintained, & clean
  There is drywall debris on the floor beneath the garbage disposal unit and the pipes connected to the unit. Grease is on the floor in the corner behind the deep fryer.
• Adequate ventilation & lighting; designated areas used
  There is no lighting (blown bulbs) in the reach-in refrigerator and reach-in freezer.
• Adequate ventilation & lighting; designated areas used
  The ceiling light tubes at the bar and in the dishwash area are unshielded. The bar's ceiling light shield was placed back on the light fixture.
