    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a BLEACH and a QUATERNARY AMMONIUM test kit.

    Observation: Openings to the exterior of the building are present along the bottom of the back door.
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.

    Observation: Observed that paper towels were not provided at the front handwashing sink.

    Observation: Observed that the following is not in good repair: Cracked floor tiles throughout facility, broken handle on the walk in refrigerator.

The purpose of this visit is to conduct a follow-up to check on the repair of the pizza prep cooler. The unit has been repaired.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Three door pizza prep 46f-48f
    Correction: Within 24 hour, repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked chicken wings 45f, shredded mozzarella cheese 45f, sliced tomato 45f all inside the three door pizza prep cooler
    Correction: Manager discarded the above foods and others based upon being held in this unit for over four hours. Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below.
  • Equipment / Good Repair / Operation (repeated violation)
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Three door pizza prep 46f-48f

    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests.
    Correction: Manager discarded house pest control product. Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination.

**Due to repeated 3dr pizza prep cooler not able to hold temperature at or below 41F, EHS will perform a follow-up on or about may 5, 2014 to confirm its fix.

  • Handwashing / Where to Wash (corrected on site)
    Observation: A food employee was observed cleaning their hands in 3-vat
    Correction: ALL food employees shall wash their hands in ONLY a designated handsink. Moved to wash hands at handsink.
  • Food Storage / Preventing Contamination
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor in walk-in cooler: tomato and sauces are on the ground.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pizza sauce (56F) on counter, cooked sausage (49F), sliced tomato (50F) in prep top and cooked chicken (50F) in 3dr pizza prep cooler.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Pizza sauce was moved to walk-in cooler and since store was open for less than 2 hours. Pizza prep cooler is not cold holding properly. Observed 54F. Maintenance is called and cooler is emptied out.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: cooked chicken wings in walk-in cooler, cooked beef in 1dr reach-in cooler.
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 3dr pizza prep cooler at 54F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. Emptied out and maintenance is called.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulb(s) in the walk-in cooler are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.
  • Controlling Pests by Eliminating Harborage Conditions (repeated violation)
    Observation: Harborage conditions exist: unnecessary items and household items stored above walk-in cooler at by back door.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sliced tomato (45F), cooked sausage (44F) on prep top of 3dr pizza prep cooler - cooler emptied out
    Correction: cooked pasta (49F), ham (49F) in 1dr reach-in cooler - discarded and emptied out
  • Ambient Air & Water Temp Meas Devices, F Scale, Accurate within 3°F
    Observation: The ambient (air/water) temperature measuring device located in the following unit is not accurate in °F: 3dr pizza prep cooler, 1dr reach-in cooler.
    Correction: Ambient air and water temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/-3°F in the intended range of use.
  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The door gaskets of the following unit(s) is damaged: 3dr pizza prep cooler.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Critical: Food-Contact Surface in Contact with PHF(TCS) Cleaned Every 4 Hours (corrected on site)
    Observation: Observed pizza cutters and pizza prep table lare not being cleaned at least every 4 hours.
    Correction: Food contact surfaces of equipment and utensils must be cleaned and sanitized at least every 4 hours to prevent contamination by bacterial growth.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F (corrected on site)
    Observation: Water from the handwashing sink was measured at a temperature less than 100°F - turned up thermostat on water heater.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
