    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food, including the date of onset of jaundice or of an illness due to Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus.     Observation: Observed a food employee clean wiping cloth rags then put on gloves and continued to prepare food. EHS instructed the PIC on when to wash procedures.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Observed two sauce containers in the WIC covered in mold. PIC removed the containers from the WIC to be discarded.
    Correction: Ensure food is safe and unadulterated.

    Observation: Observed multiple unlabeled food containers holding various powders and spices.
    Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Cloths - Wiping Cloths - Use Limitation (repeated violation)
    Observation: Wiping cloths improperly stored between use. Observed several wet clothes not being stored in wiping cloth buckets.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Food Storage - Clean and Dry Location
    Observation: Food stored on the floor or food stored less than 6" above the floor. Observed several boxes of fruit stored on the floor near the WIC.;Elevate food storage onto approved shelving with minimum 6" legs or casters.

    Observation: The prepared ready-to-eat (RTE) sliced tomatoes in the WIC and Prep unit are not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition*
    Observation: The following commercially processed ready-to-eat (RTE) foods in the WIC are not labeled with a ""consume by"" date: rice, grilled chicken, sauces.
    Correction: Discard the food at this time. Ensure that all commercially processed RTE foods are mark a "consume by" date once opened. If the food is held at 41°F or below the food shall be served, sold or discarded within 7 calendar days or within 4 calendar days if held at 45°F. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QT-10 test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Equipment - Cutting Surfaces (repeated violation)
    Observation: The cutting board(s) along the large and small prep units are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils* (corrected on site) (repeated violation)
    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch near the 3 compartment sink: stainless steel holding pan, ladle, serving spoon, and measuring cup. PIC relocated all items to the warewashing area.
    Correction: Clean and sanitize these surfaces for food contact.
  • Equipment - Cooking and Baking Equipment
    Observation: The food contact equipment surface of the pizza oven rack is observed soiled with accumulations of grime and debris.
    Correction: Clean the food contact surface of the pizza oven rack to prevent accumulations that may reduce heat transfer to adequately cook food and to prevent attract of insects when not in use.
  • Non-Food Contact Surfaces
    Observation: The following nonfood contact surface of the equipment has accumulations of grime and debris: gaskets to the RIC, outside of the lids, outside of multiple holding pans, and interior of the pizza oven.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Equipment and Utensils, Air-Drying Required
    Observation: Serving pans were found stacked while wet after cleaning and chemical sanitization above the 3 compartment sink.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Their is no hot water at the handsink near the 3 compartment sink.
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.
  • Handwashing - Using a Handwashing Lavatory (repeated violation)
    Observation: The handwashing facility located at the front of the kitchen is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the equipment preventing its use.
  • Lighting, Intensity
    Observation: Less than 10 foot candles of light was noted in the WIC.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
  • Mops - Drying Mops (repeated violation)
    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

    Observation: Observed grill cleaner being stored with clean equipment behind the 3 compartment sink.
    Correction: Containers of grill cleaner must be located in an area that is not above food, equipment, utensils, linens or single service items.

    Observation: A food employee failed to wash his hands before donning gloves to engage in food preparation. Employee was instructed to wash his hands before donning gloves during the inspection.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Correction: Clean the surface of equipment at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Critical: Water Capacity* (corrected on site)
    Observation: Water heater observed turned off or not set at the proper temperature. The water temperature at the 3-compartment sink measured at 88.6'F and 87.8'F. PIC called an associate owner of the facility to adjust the temperature of the hot water heater during the inspection. The compartment sink measured at 113'F.
    Correction: Ensure that water heater is turned on and set at proper operating temperature.
  • Handwashing - Using a Handwashing Lavatory (corrected on site)
    Observation: The handwashing facility located at the kitchen entrance is blocked, preventing access by employees for easy handwashing. PIC removed all items blocking the handsink during the inspection.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the equipment preventing its use.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose (corrected on site)
    Observation: The handwash station at the kitchen entrance is being used as a dump station. Observed accumulations of food particles in the handsink. Discussed with PIC that the handsink is to be used for handwashing only.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The walls, ceiling, and floors throughout the facility, to include the walk-in units, are noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Facility is in substantial compliance with the food regulations. Permit issued. Post permit for customer viewing.
