The purpose of this visit was to conduct a routine inspection. Environmental health specialist discussed temperature logs, glove use and cleaning schedules with verification that the cleaning has been complete.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site)
    Observation: Beverage dispensing nozzles, and enclosed components of equipment are observed with black residue.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. All nozzles and enclosed components were washed, rinsed and sanitized during inspection.

The purpose of today's visit was to conduct a complaint investigation received by the Health Department on September 02, 2014. The complainant allegedly reported unsanitary condition of the restaurant, utensils not being clean after being dropped on the floor. Employees are not changing gloves after removing trash bags and dishwasher is not working. Talk to the CFM about the complaint. Observed the kitchen facility clean, organized, and proper use of disposal gloves. The dish machine was observed shutdown by there maintence department for repairs and currently using the three compartment sink for ware washing. The complaint is not confirmed.
No violation noted during this evaluation.

The purpose of today's visit is to conduct a risk factor assessment inspection in conjunction with a complaint investigation.
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item were found hot holding at improper temperatures using a calibrated food temperature measuring device: Buffet area: mix vegetables 117F
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19.--Discarded
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Victory 2dr display cooler(1): lomein noodles, white noodles, milk
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.---Discarded
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F (corrected on site)
    Observation: Water from the handwashing sink in the kitchen was measured at a temperature less than 100°F.---82F
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.---Fixed during the inspection in the range of 110-135F.

**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within fourteen (14) days. Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

    Observation: Observed the right portion of the dishmachine drainboard in need of caulking to the wall.
    Correction: Caulk the drainboard to the wall.
  • Equipment / Good Repair / Operation (corrected on site)
    Observation: Observed the Victory glass front 2-door reach-in refrigerator near the soda walk-in with an ambient air temperature of 45F.
    Correction: Adjust, repair or replace the unit so that it is capable of maintaining ambient air and food temperatures of 42F or below. NOTE: PIC adjusted the unit to 36F.
  • Equipment / Good Repair / Components / Gaskets
    Observation: Observed the gaskets on the following refrigeration units torn:
    1. Victory 2-door upright glass front refrigerator (left) near the hood.
    2. Walk-in freezer

    Correction: Replace the gaskets. NOTE: A receipt was provided for hte request for new gaskets for the walk-in freezer, made on 8/17/14. CFM stated all gaskets would be replaced. Send a picture of hte replaced gaskets to my attention at the Health Department within 14 days.
  • Equipment / Non-food Contact / Visibly Clean (corrected on site)
    Observation: Observed the interior of the microwave oven soiled with food debris.
    Correction: Clean the interior of the microwave oven. NOTE: The unit was discarded and replaced with a new unit.
  • Floor / Wall / Ceiling / Cleanability (corrected on site)
    Observation: Observed the walls of the interior of the walk-in refrigerator 4 with holes exposing a material that is not easily cleanable.
    Correction: Plug the holes to render the walls easily cleanable. NOTE: Holes were filled with silicone during the inspection.
  • Floor / Wall / Ceiling / Cleanability
    Observation: Observed the concrete column in the dry storage room constructed of a material that is not easily cleanable.
    Correction: Suitably cover the concrete column so that it is easily cleanable. NOTE: PIC has agreed to cover the column with wall panels. Provide a picture of the covered wall column within 14 days to my attention at the Health Department.
  • Handwashing Signage at Handwashing Facilities Required (corrected on site)
    Observation: Observed the absence of a sign informing employees to wash their hands at the handsink near the hood with the rethermalizers.
    Correction: Provide a sign informing employees of the need to wash their hands at the handsink.
