Certified food manager needs to be recertified. Operator said that someone has been contacted to look at refrigerated prep table to determine why it is not holding cold enough. Advised to monitor food temperatures and move food if they cannot be held at 41 degrees F or below in this unit.

Received complaint that customer found part of insect leg in food and also saw a cockroach in the toilet room. During today's visit, EHS discussed complaint with operator. EHS saw old roach parts on rack of trays in back kitchen. There are dirty trays (not in use) and newspapers on this rack. Advised operator to remove unnecessary items from restaurant including pieces of cardboard on shelves. Advised to be sure that all food/grease is cleaned up before closing each day. Advised to contact certified pest control officer to handle this. There is a can of insect spray in the kitchen. Advised not to use this in restaurant. EHS did not see any live roaches.
No violation noted during this evaluation.
