    Observation: The person in charge failed to state the required cooking temperatures for the raw burgers. PIC was provided cooking handouts.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness.     Observation: The 3 compartment sink is not sealed to adjoining wall.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: Surfaces of the soda nozzles and ice deflector shield (corrected) were in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of equipment at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Observed a powdered cleaning chemical stored above the in use 3 compartment sink. PIC relocated the chemical to the back.
    Correction: Containers of chemicals must be located in an area that is not above food, equipment, utensils, linens or single service items.

Discussed with the Person in Charge:
1. Thank you for correcting several items during the inspection today.
2. Ensure an appropriate food thermometer and test kit for sanitizer are provided as soon as possible.
3. Person in Charge discussed proper times for hand washing with the food employee, however the employee did not wash his hands at required times on additional occasions during the inspection. A hand washing hand out was provided. Continue working with food employees regarding appropriate times and procedures for washing hands.
4. Handouts covering proper cooking, holding and reheating temperatures were left with the Person in Charge during today's inspection.

    Observation: A food employee failed to wash his or her hands after removing soiled gloves and prior to donning clean gloves and returning to food preparation.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit. Observed several cartons of raw shell eggs stored on the top shelf of the reach in cooler. The eggs were stored above items such as hot dogs and sliced and whole tomatoes.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Critical: Food Contact with Equipment and Utensils* (corrected on site)
    Observation: Observed employee leave the kitchen to remove french fries from an outdoor reach in freezer and return to the kitchen. He continued to prepare foods while wearing the same gloves. Person in Charge instructed the employee to change his gloves and wash his hands.
    Correction: Use only cleaned and sanitized utensils or equipment during food preparation.
  • Critical: Temperature, Cooking - Raw Animal Foods* (corrected on site)
    Observation: The comminuted or ground beef was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria. The hamburger was cooked to approximately 138'F prior to placing in the steam table for hot holding. The burgers were placed on the grill and continued to cook until they reached temperatures of 209'F and 198'F.
    Correction: Continue to cook the meat to heat all parts to 155°F or above for 15 seconds, 145°F for 3 minutes, 150°F for 1 minute, or instantaneously at 158°F.
  • Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition* (corrected on site)
    Observation: According to the ""consume by"" date on the commercially processed ready-to-eat (RTE) pork barbecue in the refrigerator, the food should have been discarded on 9/14/2015.
    Correction: Discard the food at this time and ensure all commercially processed RTE food is served, sold or discarded by the ""consume by"" date after opening. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: There was no temperature measuring device located in the sandwich prep unit.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

    Observation: The Person in Charge could not produce an appropriate food temperature measuring device to verify cooking temperatures of thin foods such as meat patties or fish filets.
    Correction: A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a proper test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Correction: Utilize only quaternary ammonium that meet the requirements when applying to food contact surfaces. The manufacture recommends a concentration of 200ppm.

    Observation: The following utensils were observed soiled to sight and touch: set of tongs stored at the hot dog roller hot holding unit.
    Correction: Clean and sanitize these surfaces for food contact.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the table under the grill, and gas line have an accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities in Good Repair
    Observation: Some floor tiles in the dry storage and in front of the mop sink are not maintained in good repair.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Floors in the dry storage area, behind the ice machine, and floors behind the grill line are noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
