Observed several drain flies in the facility, have pest control treat for them. Also observed the floors/walls and several pieces of equipment in need of cleaning. Put food away after deliveries and do not let it sit on the floor.

    Observation: Observed employee touch raw beef with gloves, then fail to change gloves or wash hands before handling RTE food.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Observed employee drink being stored on the biscuit making station with no lid or straw.
    Correction: Drinks must have a lid and straw and be stored in a way to prevent any possible contamination of food and food contact surfaces.

    Observation: Observed the refrigerator on the line with food around 48 degrees and egg product on the breading line at 47 degrees.
    Correction: Have maintenance look at the refrigeration unit, move food to another unit until it is fixed. Add ice to the egg product to cool down to 41 degrees.

Excessive drain flies near mop sink and randomly throughout premises, contact pest control to take care of the issue. Walk-in freezer was off still due to earlier power outage, food still in temp. or frozen. Freezer is now running fine, needs to be defrosted due to excess ice build up created during this time. 3 compartment sink faucet is leaking, needs to be repaired. Gasket on right door of refrigerator under make table is torn and needs to be replaced.

    Observation: Raw Chicken at breading station did not have ice put under it to keep cold. Egg dip for chicken was also > 41 degrees.
    Correction: Add ice to bring the temperatures below 41 degrees (Currently <2 hours out of temp).

    Observation: Cleaned dishes were soiled and stacked wet.
    Correction: Go through clean rack and make sure dishes are cleaned fully when washing. Allow dishes to air dry fully before stacking them.

    Observation: Mold/Mildew build up inside the ice machine and on the inside of lid.
    Correction: Empty machine out to clean and sanitize it. Set up a routine to clean ice machine regularly.
09/30/2014Risk Factor
10:00 AM Received call from District Epidemiologist reporting customer complaint of strong sewage odor in this facility. Henrico Fire Department employee called shortly after and reported that they had responded to a complaint of sewage odor at this facility. Fire Department stated that facility was open to the public and had sewage backup in restrooms. Fire Department did not go into kitchen as PIC at facility told them sewage had not backed up in kitchen.
11:05 AM - Arrived at facility and spoke with GM. Plumber was on site and had just finished clearing the drain line that had caused sewage backup in restrooms. Drain line in kitchen was not affected. We discussed requirement to close facility when sewage backup occurs. This information is on the back of your Virginia Food Establishment Permit (12 VAC 5-421-3910). Employee is cleaning restrooms which will be reopened to the public as soon as cleaning is completed.

Manager was aware of the complaint. (Manager that day had lost her voice, sounded hoarse so the complainant may have taken her for joking). Only male employees were making and assembling biscuits that day. The manager said she checked everyone's hands and no one had fake nails that day or missing nails. No employees observed with fake nails at the time of visit. Advise to review policy with employees (nails/nail polish ok but with gloves). Check product before serving to make sure it is unadulterated.

Note: watch temperature in make table - do not fill shredded cheese pan too full or keep below. Excellent handwashing. Significant improvement in facility sanitation and organization.

    Observation: Observed sausage improperly cooled from 11 am, still at 65-72 degrees at 2:30 p.m.
    Correction: Pans were overfilled with product. Cool quickly from 135 to 70 degrees in 2 hours, then to 41 degrees in 4 hours.
