Observation: There are not an adequate number of handwash lavatories on site. (See 12 VAC5-421-2280). HAND SINK REMOVED FROM WALL AT TIME OF INSPECTION FOR WALL RECONSTRUCTION.
Correction: REINSTALL HAND-SINK TO PREVENT POTENTIAL CROSS CONTAMINATION. Re-install kitchen hand-sink to increase the employee's opportunity to wash hands routinely.
- Grease Trap
Observation: The grease trap has been removed from 3-vat sink preventing easy access for cleaning.
Correction: RE-INSTALL GREASE TRAP TO 3-VAT SINK PLUMBING. Grease trap shall be easily accessible for cleaning and maintenance.

Observation: The sink basin at the 3-compartment sink is slow to drain in the rinse and sanitize basins.
Correction: Please contact a maintenace provider to repair or correct the problem to prevent potential cross-contamination. Plumbing systems and components shall be maintained in good repair.
- Outer Openings - Protected
Observation: The outer openings to the exterior of the rear kitchen door were not completely enclosed. (Door Sweep)
Correction: All exterior outer openings shall be completely enclosed to prevent the potential entry of unwanted pests.
- Physical Facilities in Good Repair
Observation: Wall inside mop sink basin is not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: The commercially processed ready-to-eat (RTE )tomato sauce was not discarded by the ""consume by"" date.
Correction: Discard the food at this time and ensure all previously frozen prepared or commercially processed RTE foods are served, sold or discarded by the most recent ""consume by"" date marked on the food container. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.

Observation: MUT was observed in a state of disrepair and damaged.
Correction: Repair the make-up-table to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the MUT, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

Observation: Wiping cloths improperly stored between use.
Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
- Hand Drying Provision
Observation: No disposable towels were provided at the hand washing lavatory in all three locations.
Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
