The purpose of today's visit was to conduct a routine assessment. EHS advised CFM to make copy of CFM card in the event that CFM forgets to bring card on-site. Copy of CFM card suffices as proof of card possession. Provided training on how to handle distressed cans. Severely dented cans pose a direct food safety risk as they can contain Clostridium botulinum, responsible for producing botulinum toxin. Suggested creating designated area in dry storage area for damaged items. It was a pleasure serving your facility today, I look forward to your next assessment.
- Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: AMERICAN CHEESE, SHREDDED CHEESE, MASHED POTATOES.
Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety. ITEMS DISCARDED.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568.
- Segregation and Location of Distressed Merchandise (corrected on site)
Observation: Products for credit, redemption, or return are not being stored in a separate designated area.
Correction: Store damaged and/or spoiled products in a separate and segregated area to preclude the mistaken use of these products. EHS PROVIDED TRAINING ON WHEN AND WHERE TO SEGREGATE DENTED CANS IN DRY STORAGE AREA.

This is a complaint investigation. Complainant observed 3 mice in the kitchen since July. Manager was aware of the pest problem and had several pest control here and is continuing with pest control. See attached emails requesting for additional and more frequent pest control. EHS could not obtain last pest control since reports are kept with the company and not at this facility. Pest control has put out traps and manager has sanitized the place as well as clean the carpets. EHS recommended manager to: -put all the foods in the closed plastic bins -improve housekeeping at the facility, especially in the classrooms since kids eat in the classrooms. -remove trash promptly. -get things 6" off the ground and remove harborage as much as they can -to get additional pest control -and find source of entry, block it and monitor. **Clean up water heater area of mouse droppings, put a trap and monitor activity **Fax and provide a copy of pest control report by September 11, 2014.
- Critical: Controlling Pests / Methods to Use When Pests Are Found
Observation: Methods are not being used to control pests: mice. Observed droppings in mechanical room and dead mice in the classroom.
Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. Pest report requested.
- Removing Dead or Trapped Birds, Insects, Rodents, etc. (corrected on site)
Observation: Dead or trapped mice was found on the premises on glue trap under the lockers in the classroom.
Correction: Remove dead rodents, birds, and insects from the premises to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests. Removed.

Observation: Observed that the WALL UNDER THE THREE COMPARTMENT SINK is in need of cleaning.
Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
- Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
Observation: The concentration of the BLEACH solution was measured OVER 200 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
Correction: Maintain the concentration of BLEACH solution at 50-100ppm. Verify concentration using the appropriate test kit.--EMPLOYEE DISCARDED SANITIZER SOLUTION IN SPRAY BOTTLE AND PREPARED NEW. EHS PROVIDED TRAINING.
