    Observation: Improper handwashing procedures observed. Observed the cook only rinse her hands with water after handling raw chicken. PIC spoke with the cook about proper handwashing.
    Correction: Ensure all food employees clean their hands and exposed portions of their arms with a cleaning compound in a lavatory by vigorously rubbing together the surfaces of their lathered hands and warms for at least 20 seconds and thoroughly rinsing with clean water.

    Observation: A food employee was observed cleaning their hands in a food preparation sink or a sink used for utensil washing.
    Correction: Instruct food employees to clean their hands in a handwashing lavatory. Food employees may not clean their hands in a sink used for food preparation or utensil washing.

    Observation: Dispensing utensils improperly stored between uses. Observed spoons used to dispense rice stored in a container of standing water at room temperature. PIC discarded the water.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Food stored on the floor or food stored less than 6" above the floor. Observed several food items stored on the floor in the walk-in cooler and walk-in freezer. ;Elevate food storage onto approved shelving with minimum 6" legs or casters.

    Observation: The handsink in the kitchen is no longer sealed to adjoining equipment or walls.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: The upsplash of the coffee brewer at the wait station was observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of equipment at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: The cavity of the microwave oven stored on top of the ice machine is observed soiled.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.

    Observation: Single-service or single-use articles not handled, displayed, or dispensed so that the food- or lip-contact surfaces are protected from contamination. Observed plastic forks at the wait station stored with the food-contact surface facing upward and unprotected. PIC placed the forks in the container with the food-contact portion facing down and protected.
    Correction: Handle, display, or dispense single-service and single-use articles so the food- or lip-contact surface are protected from contamination.
  • Critical: Sanitizer - Criteria/Chemicals for food contact* (corrected on site)
    Observation: Chlorine sanitizer being applied to food contact surfaces does not meet the requirements. The chlorine sanitizer measured greater than 200ppm. PIC diluted the concentration to 100ppm.
    Correction: Utilize only sanitizing agent that meet the requirements when applying to food contact surfaces.

3. PIC stated all cooked foods in the walk-in cooler were prepared this morning and will be served today.

    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit. Observed raw shell eggs stored over ready-to-eat sauces in the walk-in cooler. PIC relocated the shell eggs to prevent contamination.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Dispensing utensils improperly stored between uses. Observed rice spoons stored in a container of standing water that measured at 83'F. PIC removed the utensils from the water and placed on a clean, dry surface during the inspection.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: The cutting boards stored under the prep table and on the sandwich prep units are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: Surfaces of the soda gun nozzle at the bar were observed soiled with accumulations of grime and debris. PIC removed the soda gun nozzle during the inspection for cleaning.
    Correction: Clean the surface of the equipment at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: The nonfood contact surface of the following has accumulations of grime and debris: 1. soda gun holder at the bar 2. Container storing dry noodles 3. basil container

    Observation: Observed food employee drinking from an uncovered container in the food preparation area. Discussed with PIC that all drinks must have a lid and a straw and be stored in a manner not contaminate equipment.
    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands, (2) The container, and (3) Exposed food, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.

    Observation: The following prepared ready-to-eat (RTE) in the refrigeration unit is not properly dated for disposition: cooked chicken and fried chicken in the walk-in cooler.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination. Observed plastic bag full of raw chicken stored in contact with a plastic bag of raw shrimp.
    Correction: Separate types of raw animal foods from each other such as beef, fish, lamb pork, and poultry during storage, preparation, holding, and display by the following: (a) Using separate equipment for each type. (b) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. (c) Preparing each type of food at different times or in separate areas.
