--Observed an open jar of mayonnaise stored at room temperature. Refrigerate mayonnaise once opened. Product discarded on site.

--Sushi rice prepared yesterday had a measured pH of 4.9. Owner stated sushi rice is made daily. Elevated ph sushi rice was discarded on site and a new batch made. Proper recording of sush rice pH must be conducted every 4 hours and maintained in a written log. Procedures for the preparation and monitoring of sushi rice was left on site.

--The high temperature rinse gauge is broken (not reading higher than 120 degrees F). The actual temperature of the rinse water was measured to be over 180 degrees F. Replace the rinse temperature gauge within 14 days.
