KDA_3_50116A2 - --Brown counter top cooler: hot dogs 52 F, sliced ready to eat turkey 50 F, ambient air temperature of 50 F. The cooler door had been open during lunch, as per owner. "Corrected on site" (COS-door was closed and temperature was adjusted colder).
KDA_3_50118A2 - --An open package of ready to eat sliced turkey was held four days in the Brown counter top cooler with no consume by date label, as per owner. An open package of ready to eat hot dogs were held seven days in the Brown counter top cooler with no consume by date label, as per owner. (COS-dated).
KDA_4_204112B - --There was not a thermometer located in the Brown counter top cooler.
KDA_4_50111A - --There was loose and peeling duct tape on the front edge of the 3-vat sink.
KDA_4_60111C - --There was dirt and lint accumulation in the exhaust vent over the pizza oven.
KDA_6_20214 - --The unisex restroom door was not self closing.
KDA_6_50111 - --There were a couple of ceiling tiles with water damage over the kitchen prep table and in the restroom.
