A black hose was attached to the faucet nozzle and the end of the hose was hanging below the flood rim (this could cause backsiphonage/contamination). COS-plumber fixed the ABV valve and hung the hose up above the flood rim.
- KDA_3_30412E - --The blue plastic and metal ice scoops were being stored directly on the plastic ledge on top of the ice machine. There was dust on the ledge and it is not a sanitary surface. COS-placed scoops on a plastic bag.
- KDA_3_50116A1 - --The nacho cheese sauce in the Gehl's machine was reading at 132.3F. The person in charge said it has been in the unit for 2 days. COS-discarded.
- KDA_3_50116A2 - --In the walk in cooler there were packages of string cheese at 48.3F and 47.5F. There were packages of Deli Express Mega Omelets (egg, sausage bacon and cheese) at 47.4F, 47.6F and 47.5F. There were pizza lunchables at 48.0F, 48.4F and 48.4F. There was a package of hot dogs at 46.3F. There were packages of Deli Express Char-Broil Patty & Cheese at 47.3F, 47.5F, 46.4F and 46.9F. There were packages of Deli Express Ham, Egg & Cheese Croissant sandwiches at 47.4F, 47.2F and 47.4F. There were packages of Deli Express Sausage, Egg & Cheese English Muffin sandwiches at 47.2F, 47.4F adn 47.9F. There were packages of Deli Express Sausage, Egg & Cheese Biscuits at 47.7F and 47.5F. There were packages of Sausage & Biscuits at 48.7F, 48.4F and 48.7F. There were packages of Deli Express Italian Sub (Ham, Pepperoni & Swiss) at 47.6F and 47.1F. There were packages of Deli Express 100% Beef Cheeseburgers at 47.6F, 47.8F and 46.9F. There were packages of Deli Express Chicken & Swiss sandwiches at 47.8F, 48.2F, 46.8F and 47.7F. There was a box of skinless cheddarwurst at 47.3F. All of these items have been in the unit for more than 4 hours. The ambient air temperature of the unit towards the front door fan blowers was reading 47.7F. (The ambient air temperature of the fan blowers towards the back was reading 29.6F). COS-discarded all of the food.
- KDA_3_50118A1 - --In the walk in cooler in a plastic tote there was a package of cheddar jalapeno hot dogs that were date marked 11/4/15 (held past 7 days). COS-discarded.
- KDA_4_30111 - --The fan blowers towards the front door in the walk in cooler have an ambient air temperature reading 47.7F. The person in charge called for the unit to be fixed.
- KDA_5_20515B - --The base of the faucet located on the 3 compartment sink leaks when water is supplied.
- KDA_6_20211A - --The light fixture in the back food storage area does not have a protective shield.
- KDA_6_30114 - --There is no handwashing signs the women and men's restroom.
- KDA_6_30311A - --The far light bulb in the walk in cooler is burnt out.
- KDA_6_50111 - --There are broken floor tiles by the back exit door.
- KDA_6_50112A - --There is trash on the floor in the walk in cooler.
- KDA_7_30111A - --On the rack above the 3 compartment sink there was one bottle of Dawn liquid dish soap being stored directly next to clean utensils. COS-removed chemical. (Note: no leakage detected).
- KDA_7_30111B - --On a retail shelf there was one bottle of Pine Sol, 2 jugs of liquid laundry soap, 2 can's of Off Spray and 9 squeeze bottles of sunscreen being stored directly above several boxes and individual packs of medicine. Also on a retail shelf there was one bottle of Dawn liquid dish soap, 4 cans of Comet with Bleach and 4 bags of Cascade dishwasher detergent being stored directly above 4 box rolls of Reynolds Wrap and 3 boxes of single use tissues (Kleenex). COS-rearranged all items. (Note: no leakage detected).
