(note-there are rodent droppings on the floor outside of the non-functional walk in cooler and the door is open to the cooler) KDA_3_30212 - --Located on the shelf next to the coffee pot, there was two containers holding a white substance. The owner said the white substance was sugar and flour. KDA_4_10119 - --In the kitchen, there is un-finished wood (raw) used as shelving to store food and dishes. KDA_4_60111A - --Located in the pull-out drawer by the deli case, there was a ladle, wire whisk, and spatula stored as clean with dried food debris. Located on the food preparation table, there was a slicer stored as clean with dried food debris on the backside of the blade area. The owner said it was used 1 1/2 weeks ago. (corrected on site (cos)-utensils moved to the three compartment sink and slicer was cleaned) KDA_5_20515B - --In the kitchen, the drain line hooked up to the food preparation sink was leaking onto the floor. KDA_6_20215A3 - --In the back storage room, there was a 1/4 inch gap in between the two back exterior door. KDA_6_50111 - --There is a leak in the roof in the back storage room. The liquid is leaking into a large container. KDA_6_501111A - --In the kitchen, there were 20-25 fresh rodent droppings in the cabinet below the food preparation sink where chemicals are stored. In the back storage room, there were 10-15 fresh rodent droppings on the floor below the desk, 10-15 fresh rodent droppings on the floor near the hot water heater, and 50-75 fresh rodent droppings on the floor behind the chest freezers and shelving on the north side of the back storage room. The owner said they have a licensed pest control operator that comes out monthly to spray. She said he was last there two weeks ago and replaced bait in the bait stations. KDA_7_20111B - --In the back storage room, there were two cans of chafing fuel stored above single use food containers. (cos-chemicals moved away from food containers and oil) KDA_7_20212A2 - --In the kitchen stored in cabinet below food preparation sink, there was one can of Hot Shot Kitchen Bug Killer (EPA#9688-129-8845) that was not approved for use in a restaurant. The owner said at one time she thinks they used it in the seating area.
