- KDA_2_30114D - --Employee was eating a sandwich in the grill area. She then went and grabbed hash browns out of the refrigerator and placed them on the grill without washing her hands in between tasks. Corrected on site (COS)- educated employee about when to wash hands. Observed by KDA 13.
- KDA_3_30212 - --Located on the shelf under the prep table, there were two unlabeled containers of white powder that the employee identified as sugar and salt. COS- labeled.
- KDA_3_30412F - --Located next to the grill, there was a container of 77F water that was being used to store a rubber spatula between uses. COS- educated and moved spatula to an empty container.
- KDA_3_50118A1 - --Located in the WIC, there was a container of gravy with a date mark of 7/27. Employee said that the date on the container was correct. COS- discarded.
- KDA_3_50118A2 - --Located in the Kenmore RIC, there was a container of sliced tomatoes that had no date mark. Employee indicated that the tomatoes had been sliced on 8/10. COS- employee date marked the container of tomatoes.
- KDA_4_60111A - --Located on a hook hanging above the grill, there was a pair of tongs stored as clean that was soiled with food debris. COS- moved to wash area. Located on top of the pizza oven, there was a plastic container stored as clean that was soiled with food debris. COS- moved to wash area. Located in the storage cabinet next to the pizza oven, there were several metal pans stored as clean that were soiled with food debris. COS- moved to wash area.
- KDA_4_60111C - --There was rust on the outside of the freezer located next to the back door. There is dried grease on the hood filters, the side of the stove, and the flat top grill located in the kitchen area. Observed by KDA 13.
- KDA_4_60211E4 - --There is an accumulation of calcium build up under the rim on the inside of the ice machine.
- KDA_4_90311A - --Located in the cabinet under the counter to the right of the ice machine, there was a box and several sleeves of single use cups being stored on the floor.
- KDA_6_20211A - --Located above the coffee pot and plastic drinking cups in the waitress area, the lights are not shielded.
- KDA_6_30114 - --There is no hand washing sign posted in the restrooms. COS- provided signs.
- KDA_6_50111 - --There are stained ceiling tiles and broken floor tiles in the kitchen area. Observed by KDA 13.
- KDA_6_501114A - --Located in the pizza oven that is being used for storage, there is cooking equipment and utensils that the person in charge (PIC) said were no longer being used.
- KDA_6_50112A - --Located in the cabinets under the counter in the waitress station, there were 11 dried rodent droppings. There is an accumulation of grease on the wall next to the frier.
- KDA_7_20212A2 - --Located on the floor next to the back door, there was a container of Ortho Home Defense Insect Killer EPA# 279-9534-239. The pesticide label indicated that it is a product for home use. Employee indicated that she uses it to spray along the inside baseboards of the establishment. COS- educated on proper pest control practices and moved pesticide outside.
- KDA_7_20911 - --Located in the storage cabinet next to the pizza oven, there were bottles of lotion, toothpaste, and other assorted personal items stored with cooking equipment and utensils.
- KDA_Defacto - --Consumer advisory posted on wall in dining area. --Focus on Food Safety book Five Keys to Food Safety --Frigidaire RIC had an ambient temperature of 46F. Employee said that it was only used for condiments and iced tea.
