KDA_4_50111A - --There is duct tape on the outer edge of the deli case. KDA_4_60111C - --The following nonfood-contact surfaces have a build up of dirt, food residue, grease, and other debris: handles and hinges of the front deli case, the drawer handles on the cabinets in the kitchen, the motor under the meat wrapping table, the hand-pulls on the bread case, & the metal cart under the meat-cutting table in the kitchen. KDA_6_501112 - --A dead mouse is on a sticky-trap in the bottom utensil drawer in the kitchen, alongside a muffin tin, a mixing bowl and whisks, measuring spoons, a flour sifter, and other baking utensils. COS - removed all wares from drawer to clean and sanitize, removed dead rodent from drawer - instructed to clean and sanitize drawer before restocking with utensils/equipment. KDA_6_50112A - --The following surfaces and areas have an accumulation of soil, grease, and residue: toilet room door knobs, the duct tape on the outside edge of the deli case, the floor behind the handwashing sink cabinet in the kitchen, under the Universal reach-in-freezer, and behind all the reach-in-freezers.
