- KDA_3_30414B1 - --Dry wiping clothes were stored on top of prep table throughout the food prep area not stored in sanitizer between use.
- KDA_3_30511A1 - --Boxes of raw potatoes were stored on the floor in the back hallway area.
- KDA_3_50116A2 - --The following foods items were in the north side make table pico de gallo 45.2F, slice tomatoes 43.2F, cooked chicken 44F, sour cream 45F, corn beef 45F (ambient air 45). COS, all PHF's discarded.
- KDA_4_20211A2 - --Plastic food storage lids with holes drilled into the lids. The drilled holes were not smooth but cracked, and jagged which bacteria could harbor in the holes that were drilled into the lids.

- KDA_4_60111A - --Dried food debris was on the blade of a knife that was stored as clean on the magnetic wall strip. Dried food debris was on food storage containers that were stored under the warewash storage table as clean. COS, items removed for cleaning and sanitizing.
- KDA_4_60211E4 - --Mold was inside of the ice chamber of the ice machine.
- KDA_5_20511B - --A plastic food squeeze bottle was in the base of the handsink in the warewash area. COS, squeeze bottle removed.
- KDA_7_20111A - --A large gallon container of cleaner was stored next to single-serve drinking straws under a prep table. A spray can of glass cleaner was stored next to a bottle of ketchup, and jar of olives. COS, chemicals removed.
- KDA_7_20111B - --Gallon size cleaners containers were stored above food squeeze bottles, and funnel tops in the back hallway. COS, food contact containers removed.
- KDA_Defacto - --A opened gallon of milk was in the reach in cooler without an opened container date. PIC stated the milk was open this morning. PIC stated he did not know milk needed to be dated once opened more than 24 hours. --Chicken was thawing in running cold water 60F. --Hot water 116F, handsink. --PIC provided test kit for quat, and chlorine. --South Make table pico de gallo. 39F, sour cream 40F. WIC raw fish 36F. RIC milk 41F. RIC raw fish 39F. --Talked with the PIC concerning Sick , policy for sick employees, restriction, and exclusion. PIC stated no employee has called in sick or left. working complaining of any type of illness. --Tortilla Soup 174F, Baked potato soup 163F, bake beans 138F, mash potato 138F, gravy 152F.
