Observation: Observed food thermometer is not calibrated to ensure accuracy. Observed thermometer under prep table was not working.
Correction: Calibrate food thermometer according to manufacturer's specification as necessary to ensure accuracy. Replace thermometer.

Observation: Raw hamburger patties cold holding at improper temperatures in top compartment of sandwich unit due to over stacking of patties and leaving top cover of unit open. Stacked sliced American cheese observed resting on cutting board of sandwich unit cold holding at improper temperatures
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
- Physical Facilities in Good Repair
Observation: Roof/ceiling is not maintained in good repair in that water was observed , at the time of inspection, leaking into overhead fluorescent light fixture and then leaking onto floor behind front service counter after a heavy downpour of rain.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit. Observed raw packaged hamburger stored above tomatoes in walkin refrigerator.
Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

Observation: Backflow or backsiphonage prevention device not available on threaded faucet as required by law in that I observed a hose attached to a faucet fixture connected to detergent dispensing unit above utility sink. .
Correction: Provide a backflow or backsiphonage prevention device on all threaded faucets.
- Critical: Toxics - Storage of Toxic Containers* (corrected on site)
Observation: Containers of paint are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed 2 gallons of paint stored next to single service items and carbonated drink mixing unit next to rear door.
Correction: Containers of @HAZARDOUS PRODUCT@ must be located in an area that is not above food, equipment, utensils, linens or single service items.
