Today a ROUTINE INSPECTION of your facility was conducted. Thank you for accompanying me during this inspection.
NOTE: Employees were observed washing hands and using gloves when handling food. Employee Health Policy guidelines and training is on-line and manager is able to pull up documentation of this training on the computer.

  • Light Bulbs / Locations where Shielding is Required (corrected on site)
    Observation: Light bulb in the Walk in Refrigerator has cracked /broken shielding .
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. AS PER MANAGER A NEW SHIELD HAS BEEN ORDERED FOR THE LIGHT BULB.

On site visit made following complaint received of raw sewage seeping up from the surface of the concrete pad next to the building through a crack in the surface. Observed that clean up complete outside near the drain and grease trap area. As per CFM there was a back up of the grease trap which flowed our through the storm drain. Plumbing service was called immediately and cleaned the back up in the sewer manhole downstream of the grease interceptor. EHS flushed toilets and observed no back up or overflow. Document provided that has been attached to file. Complaint closed.

    Observation: Observed the handsink at the BACK KITCHEN AREA being used BY STAFF FOR WASHING PUREE SQUEEZE BOTTLES.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. SPOKE TO STAFF ABOUT USING HAND WASH SINK ONLY FOR HAND WASHING. ASKED MANAGER TO TRAIN EMPLOYEES ON HAND WASHING AND WHEN TO USE HAND WASH SINKS .

  • Microwave Oven Interior & Door Seals Cleaned Every 24 Hours
    Observation: The (cavity, door seal) of the oven opposite the 1 DR Hoshizaki upright refrigerator is observed soiled with visible grease and debris. .
    Correction: The interiors of all microwave and ovens including cavities, door seals, floors, walls, and ceilings shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure. ASKED MANAGER TO CLEAN THE INSIDE OF THE OVEN.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

Site visit made to investigate complaint received on 09-May 13 regarding a smell near the counter area and gnats . Could not verify strange smell, complaint on flies was valid.
Observed several flies that appear to be drain flies at the front cashier area. CFM, Khurram Chowdry informed me that there was a block and back up in the drain in front of the store under the cashier counter and the two bathrooms resulting in standing water . The block was snaked on 5- May 2013 by G and B Plumbing ( Invoice produced). Following this caulking was completed at the drain under the cash counter . Work was not performed correctly and will be redone. Western Pest control will perform pest control service to exterminate for the flies.
All work including professional pest control to be completed within 5 business days and invoice faxed to Health Dept. Attn: Suja at 703-385-9568 or emailed at suja.warrier@fairfaxcounty.gov

    Observation: Employees observed working in the food service area without proper hair restraints.OBSERVED MOST EMPLOYEES AT THE ESTABLISHMENT NOT WEARING THE HAIR RESTRAINT AT THE START OF THE INSPECTION.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff. CORRECTED
  • Critical: Package Integrity & No Dented Cans/ Specifications for Receiving (corrected on site)
    Observation: The following food item from damaged packaging were found offered for sale or service:OBSERVED DENTED CANS BEING STORED ON SHELVES WITH OTHER CANS AND ITEMS.
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit. CORRECTD: CFM REMOVED THE DENTED CANS FROM THE SHELVES.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item was observed cold holding at improper temperatures using a calibrated food temperature measuring device: OBSERVED SHREDDED CHEESE HELD AT 47F IN THE SANDWICH UNIT ( FRONT )
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.CORRECTED
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold holding equipment:OBSERVED BOTH SANDWICH UNITS, OUTDOOR UNIT, BEVERAGE AIR REFRIGERATOR , WALK INS AND MEAT COOLER DID NOT HAVE A TEMPERATURE MEASURING DEVICE.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. OBSERVED CONCENTRATION OF SANITIZING SOLUTION IN THE BUCKET NEAR THE CASH REGISTER TO BE 0 PPM WITH A QUAT TEST KIT.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.CORRECTED
