Please email the EHS when the hot water in women's room is fixed.
REMINDERS and RECOMMENDATIONS:
1) Please make sure that the ice dispenser nozzle at the soda machine is also cleaned regularly.
2) Do not use the spray hose in the middle sink because that dilutes the sanitizer solution.
3) Kindly adjust thermostat for the 1DR prep refrigerator to 41F or less. Recommend all refrigerators at 37F during summer time.
4) Use test strips to check the sanitizer solution.

  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Thawing / Approved Methods (corrected on site)
    Observation: Observed the following food thawing using an improper method: Observed grilled chicken thawing on shelves near ice machine.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: The handwashing sink at women's room is equipped with separate cold and water faucets rather than a mixing valve or combination faucet. Observed no hot water.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
  • Physical Facilities Good Repair
    Observation: Observed that the cove molding and floor tiles on the left side of the smoothie refrigerator is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. Note: They have placed a work order and someone is going to fix it today.

    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit. Note: EHS provided one.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the quaternary ammonia solution was measured at more than 500 ppm in wet wiping buckets. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of quaternary ammonia solution at 200 ppm. Verify concentration using the appropriate test kit. Note: Make new solution.

    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Storing ice scoop in ice dispenser bin.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: Observed food employees using improper handwashing procedures. Witnessed workers washing hands in cold water and touching faucet handles with wet hands before drying.
    Correction: Food employees shall wash their hands and exposed portions of their arms by: 1) discarding used gloves

The purpose of this visit was to conduct a routine risk inspection. Water heater information: AO Smith DEL 50 110. Quaternary ammonium sanitizer in 3 vat sink: 300ppm. PIC to send signed employee illness reporting agreements to Health Department within 14 days. PIC to provide service invoice to Health Department for repair of slow draining sink within 14 days. NOTE: PLEASE REMEMBER TO PROVIDE SERVICE INVOICE FOR SINK and SIGNED EMPLOYEE REPORTING AGREEMENTS TO HEALTH DEPT. WTHTIN 14 DAYS. AS REQUIRED ABOVE.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the PIC advised that the employee reporting agreements had been signed, but were not accessable. NOTE: PIC will FAX copies of employee reporting agreements within 14 days.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: Diced chicken in hot hold unit: 81 degrees F. NOTE: Temperature measured 138,146 degrees F 20 minutes later.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: both 3 dr prep coolers.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Handwashing Sink / Accessible at All Times (corrected on site) (repeated violation)
    Observation: Two hadwashing stations blocked, preventing access by employees for easy handwashing. NOTE: PIC removed dish detergent from handsink, and trash can blocking other handsink.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
  • Plumbing / Maintained in Good Repair
    Observation: The handwashing sink at the 3 dr smoothie prep cooler was draining slowly.
    Correction: A plumbing system shall be maintained in good repair.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site)
    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.
