This visit was made to conduct a routine food safety evaluation. The Person in Charge was very helpful throughout the visit and very proactive in making corrections. Thank you.
The following issues require attention:
1. At the beginning of the visit observed that the high temp dishwashing machine in the upstairs prep area was not reaching correct final rinse temperature of 180°F as per manufacturers instructions. Employees corrected this during the inspection by changing the water in the machine and running the machine multiple times. Please monitor this machine to ensure that it is reaching its designated final rinse temperature and that water is changed as per instructions on the unit.
2. Observed various Time/Temperature Control for Safety Foods (TCS foods) on top of the cold prep unit #7/8 that were being held above 41°F. Please monitor this unit and employees to ensure that all TCS foods are held cold at 41°F or below.

  • Utensils That are In-Use / Between-Use Storage
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Ice scooper was stored on top of ice machine in upstairs food prep area with lip of scooper in contact with non food surface.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. Discussed this with Person in Charge and he will reattach ice scooper holder which had become detached from the side of ice machine.

    Observation: The following food item(s) were measured at improper temperatures: sliced turkey at 45°F in unit 4, cut tomato (50°F), ham (45°F) and crab meat (48°F) in unit 7. Milk at 47°F in unit 17 in dry storage room.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. Person in Charge corrected this by discarding the sliced turkey, cut tomato, ham and crab meat. Milk was moved into the walk in refrigerator which had temperature of 39°F.
  • Mechanical Warewashing Equipment, Hot Water Sanitize, Final Rinse Temp (corrected on site)
    Observation: The water temperature in the manifold servicing the high temperature warewashing machine was below the minimum allowable level of 180ºF. The warewashing machine failed to trigger the thermolable temperature of 160°F on a plate put through the machine at the beginning of the visit.
    Correction: In a mechanical operation the temperature of the fresh hot water sanitizing rinse as it enters the manifold may not be more than 194°For less than 165°F for a stationary rack, single temperature machine or for all other machines, 180°F. Establishment employees changed the water in the machine and at the end of the visit the machine was functioning properly with a final rinse temperature of 180°F and the thermolable was triggered when placed on a plate run through the machine.

    Observation: The prepared ready-to-eat (RTE) rotisserie chicken, squash & coconut soup in the refrigeration unit is not properly dated for disposition. Food were discarded during the inspection.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: walk-in refrigerator. Foods were covered during the inspection.
    Correction: Foods shall remain covered at all times.
  • Critical: Cooling Potentially Hazardous Foods (corrected on site)
    Observation: Observed that the following potentially hazardous foods were not cooled from 135°F to 41°F within a total of 6 hours: rotisserie chicken (prepared 5/31/15) and squash & coconut soup (prepared 5/26/15) were 45°F. Foods were discarded during the inspection.
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within 2 hours and from and 135°F to 41°F within a total of 6 hours to prevent the growth of harmful bacteria. Foods that have been temperature abused must be discarded.
  • Equipment / Durability
    Observation: The following equipment is not designed and constructed to be durable: home style microwave oven. Replace homestyle microwave oven with a commercial grade microwave oven.
    Correction: Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.
  • Equipment / Food-Contact / Visibly Clean
    Observation: The following equipment/utensils were observed soiled to sight and touch: the interior of refrigeration units #2 and #4
    Correction: the microwave ovens,and the interior of the warmer units.
  • Critical: Sanitize / Hot Water / Mechanical/Thermolabel Test Verified
    Observation: Due to improper operation of the mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 4-501.112) Use the 3 compartment sink to effectively sanitize the equipment after it washed and rinsed at the mechanical dish machine.
    Correction: Hot water warewashing machines shall achieve a utensil/equipment surface temperature of at least 160°F as measured by an irreversible registering temperature indicator. The thermolabel provided on the equipment/utensils shall turn black if the warewashing machine is properly heat sanitizing as required. If the thermolabel does not turn black, the warewashing machine shall not be used. Employees will have to set up the 3-vat sink to manually wash, rinse, and chemically sanitize all equipment, utensils, dish ware, glass ware, etc. A follow-up inspection will be conducted within 24 hours to ensure the machine has been repaired.

    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 4-501.114) The bar dish machine is not effectively sanitizing. Do not use the bar dish machine to sanitize use 3 compartment sink to effective sanitize glassware from the bar.
    Correction: Cease use of the chemical sanitizing warewashing machine at this time. The warewashing machine shall not be used unless it is properly repaired and chemically sanitizing equipment, utensils, dish wares, glass wares, etc. at a concentration of at least 50-100ppm of chlorine for the required amount of time as specified by the warewashing machine's data plate.

    Observation: The handwashing station at the soup cooking are is being used as a dump station (parsley).

Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc.

    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: beef was improper stored above fish in the walk-in refrigerator, and chicken was stored above beef. The Chef reorganized the food during inspection.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature).

This visit was made to conduct a routine inspection. Person in charge and food employees were very helpful and knowledgeable. The following items require attention:
1. Repair condenser unit to prevent ice build up in the walk-in freezer.
2. Make sure to indicate the date when the last shellstock from the container is sold or served on shellstock tag. Provide a copy of original shellstock tags when removing from their tagged or labeled container.
Repeat violations are subject to civil penalties.

  • Certified Food Manager: Certificate Process
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card.
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be faxed to the Environmental Health Division at 703.746.4919. Failure to provide a copy of the photo identification card may result in further enforcement action.

    Observation: The following food item(s) were measured at improper temperatures: the steak in unit #3
    Correction: the goat cheese, mozzarella cheese sliced ham & liquid eggs in unit # 7
  • Equipment / Good Repair / Operation
    Observation: The following mechanical refrigeration unit is not currently operating as required to cold hold food at a temperature of 41°F or less: units # 7 and #13 are not holding PHF at the required temperature. The walk-in freezer is not operating properly and is causing thick frost on all food items. Repair the refrigeration units immediately. Repair the walk-in freezer in 10 days.
    Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators.
  • Critical: Sanitize/Chemical/Manual & Mechanical/Concentration & Exposure Time
    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 4-501.114) The bar dish machine is not sanitizing.
    Correction: Cease use of the chemical sanitizing warewashing machine at this time. The warewashing machine shall not be used unless it is properly repaired and chemically sanitizing equipment, utensils, dish wares, glass wares, etc. at a concentration of at least 50-100ppm of chlorine for the required amount of time as specified by the warewashing machine's data plate.
  • Controlling Pests by Routine Inspection of Premises
    Observation: The premises is not being routinely inspected for the presence of insects, rodents, and other pests. Observed numerous flying bugs in the kitchen.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Routinely inspect the premises to control the presence of pests on the premises.

This visit was conducted to complete a routine food safety evaluation.
1. You must ensure that steaks are not placed above ready to eat items in line coolers. This is the second time that this has been observed.
2. Ensure that salad items on the staff buffet are pre-chilled before display.
3. Ensure that all equipment is washed, rinsed and sanitized prior to storage.

  • Molluscan Shellstock/Maintaining ID/Tags Attached until Container Emptied (corrected on site)
    Observation: Tags missing from the molluscan shellfish containers.
    Correction: Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty. (Discarded)
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site) (repeated violation)
    Observation: In two separate refrigerators raw steak was observed over ready to eat items.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were measured at improper temperatures: Tuna was observed at 60°F in the staff buffet
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. (Discarded)
  • Date Marking of RTE, PHF(TCS) Prepared On-Site, Stored at 41°F or less
    Observation: The following ready-to-eat, potentially hazardous food in the refrigeration unit is not used within 24 hours of preparation and is not properly dated for disposition within 7 days at 41°F: Short rib prepared two days ago.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods (time/temperature control for safety food) that are prepared on site and held at 41°For below shall be clearly labeled and date marked with a "USE BY" not exceeding 7 TOTAL DAYS. If the food is held for only 24 hours, no date marking is required. For example, if foods like leftover soups, chili, tuna salad, potato salad, pasta salad, etc. are prepared on @DATE@ they shall be date marked with a "use by" date not exceeding @DATE@.
  • Cutting Boards / Resurface / Discard (repeated violation)
    Observation: The cutting board(s) along the line area are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: Lemon wedger.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Items were observed stored in the hand washing sinks in the staff cafeteria and the dishwashing area.
    Correction: A hand washing sink may not be used for purposes other than hand washing.

This visit was conducted to complete a routine food safety evaluation:
1. Ensure that the cold holding units on the line maintain potentially hazardous food at 41
°F
or below.
2. Remind team members to ensure that raw steaks are stored below ready to eat items on the line.
3. The CFM was knowledgeable on employee health, and quick to resolve issues raised.

  • Molluscan Shellstock/Maintaining ID/Recording the Date Sold
    Observation: The date on which the last shellstock from the container is sold or served was not recorded on the tag or label.
    Correction: The date when the last shellstock from the container is sold or served shall be recorded on the tag or label.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw steak was observed stored above sliced ham in the upright line cooler.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Serving utensils at the bar were observed in standing water.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were measured at improper temperatures: Crab observed at 52°F, Lobster at 49-51°F, Ham at 54°F and Cut Tomatoes at 54°F in 2 line coolers.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. (Items relocated)
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board(s) in the kitchen are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Physical Facilities Good Repair
    Observation: The two line cold rail units were observed to have an air temperature of 50°F, indicating that they are not working correctly.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
