The purpose of today's visit is to conduct a risk factor inspection.
  • Critical: Consumer Advisory, Disclosure Provided
    Observation: Asterisking (*) missing for the salmon dishes.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods as raw/undercooked or identification of the animal-derived foods by asterisking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.

This inspection was conducted after the fact of a change of ownership. All timelines are to be met as designated.
**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding items which have not been designated with a specific compliance timeframe are to be corrected no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.** Email or fax a copy of the specification sheet for any new equipment to the health department before making any purchases.
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review. Hoods were cleaned in August 2015.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review. Grease traps are cleaned quarterly.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

  • Critical: Consumer Advisory, Disclosure Provided
    Observation: There are numerous items on breakfast, lunch and dinner menus which are indicated with an asterisk(*). The advisory has only the reminder statement.
    Correction: Add the disclosure statement to all menus, as applicable, within 14 days. Forward electronic versions of modified menus to the heatlh department within 14 days.
  • Sanitizer Test Kit Required
    Observation: There is no test kit located at the bar for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit to be stored and used at the bar.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Walkin refrigerator in Garmanger kitchen (#1)
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. Forward service order to the health department within 48 hours.
  • Critical: Sanitize/Chemical/Manual & Mechanical/Concentration & Exposure Time
    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning at the bar. (see 4-501.114)
    Correction: Service the dishwasher at the bar. Forward the completed work order to the health department within 48 hours.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: The water at the handsink in the garmanger kitchen was measured at 75F. Hot water must be available at 100F at all handsinks.
    Correction: Adjust mixing valve to provide 100F mixed water at the handsink in the garmanger kitchen.
  • Critical: Handwashing Sinks / Numbers By Law
    Observation: There is not an adequate number of handwashing sinks lavatories on site. There is not a handsink at the bar in the lounge.
    Correction: Install an NSF approved handsink, soap dispenser, paper towel dispenser and handwashing sign in an appropriate area at the bar. The installation must be completed within 30 days. Forward a picture of the handwashing area to the health department within 30 days.
  • Critical: Backflow Prevention, Device / When Required
    Observation: Observed the lack of an approved backflow prevention device on the following water supplies: coffee maker to the left in the kitchen
    Correction: icemaker in the kitchen.
  • Hand Drying Provision / Individual, Disposable Towels
    Observation: Observed that paper towels were not provided at each handwashing sink.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing - coffee bar.
  • Handwashing Signage at Handwashing Facilities Required
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees - coffee bar
    Correction: garmanger kitchen.
  • Physical Facilities Good Repair
    Observation: Observed that the ceiling tiles above the main dishwasher are mildewed.
    Correction: Replace the mildewed tiles. Tiles have been ordered, per engineer.
