    Observation: A food service employee was working in the food service establishment without having a valid food service card.
    Correction: A food service employee shall have a valid food service card prior to working in the food service establishment.     Observation: Ice scoop observed being stored on surface that was not wash/rinsed/sanitized.
    Correction: Store ice dispensing utensil in one of the following manners: in ice with handle above the top of the ice within the container or equipment that can be closed, or in a clean, protected location.
  • Cloths - Wiping Cloths; Use Limitation
    Observation: Improper use of wet wiping cloths.
    Correction: Use clean wet wiping cloths for wiping spills from food-contact and nonfood-contact surfaces of equipment. Store wet wiping in a chemical sanitizer at the proper concentration between use.

    Observation: The prepared ready to eat food in the refrigeration unit was not properly dated for disposition.
    Correction: Mark the name and consume by date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The prepared food in the freezer was not observed date marked.
    Correction: Mark a 24 hour consume by date once the food is thawed. Prior to freezing Indicate the length of time the food was held refrigerated which is 7 days or less at 41°F. When the food is removed from the freezer indicate the date by which the food shall be consumed which is 7 days or less at 41°F minus the refrigeration time before the food was frozen.

    Observation: Multiple non food contact surfaces in bar areas and in kitchen are in need of being cleaned of grime and food debris.
    Correction: Clean non-food contact surfaces as often as necessary to prevent build up grime and food debris.

    Observation: No food temperature measuring device was observed on premises.
    Correction: Obtain at least one food temperature measuring device so employees can properly monitor the temperature of food items in the establishment.

    Observation: Floors and walls were observed not smooth and easily cleanable throughout areas of facility.

    Observation: Several areas observed with coving peeling away from wall. Several walls were observed with gaps where walls should meet.
    Correction: Cove floor wall juncture to no larger than a 1/32 inch space. Seal gaps in walls.

    Observation: Women's toilet room door is not tight fitting or self closing.
    Correction: Provide toilet room door that is tight fitting. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.

    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees

    Observation: Toilet tissue was not present at toilet in mens restroom.

    Observation: Flies were noted in the establishment

Animals carry disease-causing organisms and can transmit pathogens to humans through direct and/or indirect contamination of food and food-contact surfaces. Animals shed hair continuously and may deposit liquid or fecal waste, creating the need for vigilance and more frequent and rigorous cleaning effort.

Provided 2013 FDA Food Code changes, temperature charts, certified manager and food handler class schedule and new employee health policy.Facility is well maintained and clean. Will enquire with office staff regarding permit.

    Observation: The one liter bottles of Hennessey, Jim Beam, Dewar's, Jack Daniels whisky's and Fire Fly vodka is unsound or adulterated. Dead fruit flies observed in all bottles.

    Observation: Animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in Ready-To-Eat form or as an ingredient in another Ready-To-Eat Food. Steak observed on the menu with no disclosure or reminder. Said food item also observed in the refrigerator.
    Correction: The Permit Holder shall inform Consumers of the significantly increased Risk of consuming such Foods by way of a Disclosure and Reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.

    Observation: Observed accumulations of grime, pink and brown colored mold and debris on the following food contact surfaces: the white plastic middle panel of the ice machine in the kitchen. Said surface was in direct contact with ice intended for customer consumption.
    Correction: Clean and sanitize these surfaces for food contact.

    Observation: Premises are not being routinely inspected for evidence of pests. Accumulation of mice droppings (approximately 30) observed on the floor under a storage container in the rear storage room. Mice droppings also observed in restroom that is under construction. Several holes and openings observed in the walls and floors of the storage room. Crevice observed in the wall of the restroom
    Correction: Inspect premises on routine basis for the evidence of pests. . Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

    Observation: Toilet room doors are being kept open in the rear bar area.
    Correction: Keep toilet room doors closed except when cleaning or during maintenance operations to prevent insect and rodent entrance and the associated potential for the spread of disease.

    Observation: Working containers of cleaning compounds are not properly labeled in the kitchen and the pool area bar.
    Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.

    Observation: Pesticides are not being applied with a certified applicator. Can of pesticide observed stored under the bar area.
    Correction: Restricted used pesticides are to be applied by an applicator certified as defined in 7 USC 136(e) Certified Applicator, of the Federal Insecticide, Rodenticide Act, or a person under the direct supervision of a certified applicator.
