Observation: The nonfood contact surface of the floor of the walk in freezer has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: Plumbing connections under the three compartment sink are leaking. The handle for the cold water at the handsink is broken.
Correction: Plumbing systems and components shall be maintained in good repair.

Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. There is a gap at the back door.
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria. Operator discarded during the inspection. Discussed botulism and why holding fresh garlic and oil mixtures could have the botential for the production of the botulism toxin.

Staff corrected during the inspection.

Leaving cooling logs for the operator to track cooling methods of fillings. Please provide a copy to the heath department once processes are verified. Coving to be installed in the kitchen with in 10 days. Operator to obtain test strips for quantinary ammonia. Self closure device to be installed on the bathroom door.
