The purpose of today's visit was to conduct a risk factor assessment. The following violations have been observed for the second time:
1. Improper cold holding of potentially hazardous foods.
The following suggestions have been made as a means to incorporate Active Managerial Control:
1. Create and monitor temperature logs for all items being cold held, paying close attentions to items on prep counter being held in cold wells.Temperature logs provided (English). Also suggested the use of Time as a Public Health Control for items being cold held at prep counters however Certified Food Manager prefers to monitor using temperature only.
2. Review and retrain employees on employee health policy regularly
3. Train employees to leave items that are cooling unwrapped until cooled completely in order to ensure items are being cooled properly.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: ON PREP COUNTER- BEAN SPROUTS, CUT TOMATO (44F, 51F)
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. ITEMS PLACED IN WALK-IN COOLER

This risk factor assessment performed concurrently with complaint investigation (complaint received 16 June 2015). EHS provided additional training regarding:
*Thawing methods
*Manual dishwashing and sanitizing, drying methods
*Date marking
*Storage of employee food and beverages
*Handwashing and glove use
Active managerial controls suggested to CFM include routine checks of date markings on prepared foods, and of washing/sanitizing procedures.

  • Duties / Monitor Sanitizing Practices and Procedures (corrected on site)
    Observation: Employees are not properly sanitizing cleaned multiuse equipment and utensils. Sanitizing basin of three vat sink was observed soiled. Drinking glasses ready for service to customers and cooking utensils/equipment were observed soiled to sight or touch.
    Correction: The Person in Charge or certified food manager shall train his/her employees to properly sanitize all equipment, utensils, and surfaces after being cleaned using either an approved sanitizer at the adequate concentration or heat sanitization at the proper temperature to destroy pathogenic bacteria. EHS reviewed manual dishwashing, sanitizing methods, and supporting public health rationale with CFM.
  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils, or after engaging in any activity that could contaminate the hands. An employee was also observed rinsing single-use gloves.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. EHS reviewed proper handwashing and glove use with CFM and employees. Employees properly washed hands.
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. An unsealed container of employee food was also stored where it could easily spill onto a food prep surface.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items. Employee foods and drinks were relocated where they would not contaminate foods or food contact surfaces.
  • Gloves/Use Limitation/No Re-Use/Use For Only One Task
    Observation: Single-use gloves worn during multi-tasked food preparation. Gloves were rinsed in handsink, and employee attempted to reuse them.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: multiple soups and sauces in walk-in refrigerator.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. EHS reviewed date marking with an employee and CFM. Foods were properly marked.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: cooking pots and pans in storage, drinking glasses, knives in storage.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. Soiled dishes were relocated for washing and sanitizing. EHS reviewed proper washing and sanitizing procedures with CFM. Remember to change wash/rinse/sanitize water when it is visibly cloudy or soiled. Keep basins of three vat sink clean to allow proper and effective washing and sanitizing.

EHS provided additional training on:
* Proper set up and use of the three vat sink - Set up all three compartments properly before beginning to wash dishes. Keep the sanitizing compartment as clean as possible to ensure the effectiveness of the bleach sanitizer
* Cold holding using ice baths - Remember that the level of ice and water should be about the same as the level of the food in the containers.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: Garlic and oil mixture (48 F) and bean sprouts (53 F) stored in ice baths were observed cold holding at improper temperatures.
    Correction: Potentially hazardous foods must be held at a temperture of 41 F or colder at all times. EHS provided training on proper use of ice baths for cold holding foods. Remember that ice level should be as high as the level of the food. Foods were relocated to freezer for rapid cooling.
  • Critical: Cold Holding of Raw Shell Eggs at Ambient Temperature of 45°F or less (corrected on site)
    Observation: Observed raw shell eggs stored at room temperaure.
    Correction: Shell eggs that have not been treated to destroy Salmonella shall be stored in refrigerated equipment that maintains an ambient temperature of 45°F or less. Eggs were relocated to walk-in refrigerator.
  • Critical: Backflow Prevention, Air Gap / When Required (corrected on site)
    Observation: Observed mop sink faucet with attached long hose did not have a backflow prevention device.
    Correction: Disconnect hose from faucet when it is not in active use or install proper backflow prevention device. Hose was disconnected.
  • Plumbing / Maintained in Good Repair
    Observation: Observed leak from faucet at mop sink.
    Correction: Maintain plumbing in good repair.

    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. Sanitizer was made and wet wiping towel were placed.

Observed the sanitizing solution in the wet wiping bucket with a concentration of above 200 ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.

    Observation: The following food items that are not easily identified by appearance were observed without a label: sauces, spices and herbs on prep cart located by the wok area
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.

    Observation: Manufacturer containers were observed reused for the storage of foods.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.

    Observation: The following equipment/utensils were observed soiled to sight and touch: plates store under prep table closer to rice cooker and knives stored in between equipment.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.

    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: clean inside of True freezer, gaskets on Superior prep unit and black cart located beside of True freezer.
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

    Observation: Walls throughout the kitchen are observed with dirt and food debris.
    Correction: Clean all floor, walls and ceilings to prevent contamination to food and non food contact surfaces.

    Observation: Observed personal clothes are not located to protect food, equipment, linens, and/or single-service items from contamination.
    Correction: Relocate personal clothing to prevent the potential contamination of food, food equipment and utensils, linens, or single-service items.

    Observation: Observed medicines are stored in such a way that they could contaminate clean plates in the kitchen under the prep tables.
