Note: The string cheese and yogurt delivered this morning were at 43F and 46F respectively. This is an indication of food not delivered at required temperature. Please check receiving temperatures of potentially hazardous foods and make sure it is at 41F or less. The manager has the right to refuse the delivery if it is not at required temperature.
Note: The facility is newly constructed and the handsink in kitchen is not connected to the kitchen water heater. The kitchen manager is working with the contractor to get it fixed. Please send an email to the EHS as soon as that is fixed.

This is pre-opening for remodeling completed at this food facility. Permission is granted for stocking of food and utensils. Approval by Health Department is hereby granted. No equipment replacement, addition, or changes are allowed without approval by the Health Department.

08/27/2015Pre-Opening
Note: The building is under construction and there is no AC on this side of the building for the last 2 days. The refrigerators were marked as 40, 38F at the beginning of the day, but it is above 41F during inspection due to lunch rush and elevated temperatures inside the kitchen. The manager called and requested the technician to come out today and adjsut/fix the refrigerators.
Please fax or email a copy of work order for adjusting/fixing the refrigerators within 5 days. Do not use the McCall 2DR upright refrigerator and milk cooler on left to hold any potentially hazardous foods (PHFs) until capable of holding PHFs at 41F or below.
Kindly repair these 2 units within 5 days and adjust the temperature for all the refrigerators/coolers to 34F during summer time, especially since there is not AC.
The manager will store all the extra PHFs in freezer and other working coolers. It was hard to get ambient temperatures for the upright refrigerators as they are in use.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: shredded cheese-salad box 50F--cold plate. Yogurt 47F--2DR upright refg right. Sliced cheese 44F--2DR upright left.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Yogurt discarded and salad put back inside the refrigerator.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Milk cooler on left 50F and McCall 2DR upright refg left 47F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. Note: The kitchen has no AC and the unit stays open, however, get technician to check and adjust the unit to 35F during summer time. Move all the milk to cooler on right.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit is damaged: milk cooler left
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

    Observation: The food temperature measuring device was found out of calibration in the range of use.Observed facilities thermometer at 18F when put in ice and cold water and a calibrated thermometer at 32F
    Correction: Food thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. It is suggested that food thermometers be calibrated at least once a week in a cup completely filled with ice and cold water to a temperature of 32°F or calibrated in a pot of boiling water at 212°F.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Water from the handwashing sink behind the milk cooler was measured at a temperature less than 100°F.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
  • Physical Facilities Good Repair
    Observation: Observed that the wall behind the rethermalizer exhibits damage and exposed dry wall.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item were found hot holding at improper temperatures using a calibrated food temperature measuring device:Observed sweet potatoes at 119F on the service line - REHEATED TO 180F
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control".
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids
    Observation: Observed the recycling bin covers were broken.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Physical Facilities Good Repair
    Observation: Observed that the wall behind the oven was crumbling.
    Correction: Repair the physical facilities.
