The purpose of today's visit is to conduct a routine inspection. Observed good employee hand washing throughout inspection.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Both light bulb(s) under the hood above the pizza prep unit and pizza oven are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.

The purpose of today's visit was to conduct a Risk Factor Assessment. Observed frequent hand washing by food employees. Observed a very good date marking system. All items using Time as a Public Health Control were labeled with the appropriate discard time. Observed food employees taking temperatures of the food.
No violation noted during this evaluation.

    Observation: The door gaskets of the following unit are (damaged): Undercounter refrigerator at service line
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. MANAGER STATES GASKETS WERE ALREADY ORDERED TO REPAIR THE REFRIGERATION UNIT.

The purpose of today's visit was to conduct a risk factor assessment inspection.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located at the front is blocked, preventing access by employees for easy handwashing.-----------Observed a stack of several customer trays blocking the front hand sink near drive thru.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use. MOVED

The purpose of today's visit was to conduct a routine inspection. Manager on duty agreed to email/fax a copy of the CFM photo card no later then the beginning of June 2014.
  • Certified Food Manager/Certificate Process (repeated violation)
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

* The purpose of today's visit was to conduct a routine inspection.
* All non-critical violations should be corrected within 30 days and no later than by the time of the next inspection.
* Employee health policy given to the PIC at the time of today's inspection.

  • Duties / Inspect Foods upon Receipt
    Observation: Based on a discussion with the PIC/CFM, food employees are not inspecting the food upon receipt to verify proper temperature and intact packaging. Based on discussion with PIC there are keydrop food deliveries taking place without inspection from PIC/CFM.
    Correction: The Person in Charge or certified food manager shall designate employees to inspect the food upon receipt from the licensed vendors. The employees shall visibly observe the foods as they are received to determine that they are from approved sources, delivered at the required temperatures of 41°F or less, 135°F or above, or frozen solid, the temperatures of the foods are documented in a daily log book, and protected from contamination.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Discussed with manager, and copy of the employee health policy was given to the manager.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: Marinaira Sauce.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. Discussed with manager, and marinaira sauce was rapidly reheated to 165 F.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required (corrected on site)
    Observation: Metal and plastic dishware were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food. Discussed with manager.
  • Certified Food Manager/Certificate Process

The purpose of today's visit was to do a follow-up inspection to verify compliance status of violations observed during the pre-opening inspection. The remaining violations are to be corrected by the time of the first routine inspection, within 30 days. Contact me if any questions arise.
  • Ventilation Hood System / Drip Prevention (repeated violation)
    Observation: The hood for the taco bell area is missing a drip pan to prevent the hood system from dripping onto food contact surfaces.
    Correction: Obtain and install a drip pan at the hood. NOTE: CFM said owner will purchase and install the new drip pan by the time of the first routine inspection.
  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The gaskets on the walk-in refrigerator are torn.
    Correction: Replace the gaskets. NOTE: CFM stated that the gaskets are on order.
  • Plumbing / Maintained in Good Repair (repeated violation)
    Observation: The following plumbing systems are not maintained in good repair:
    1. Handsink near the drive through with a small leak in the piping beneath the sink.
    2. Toilet in the men's room which is clogged and not properly functioning.

    Correction: Repair the plumbing fixtures. NOTE: Handsink has been taken out of order. CFM stated handsink will be replaced.

**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected by November 22 at which time a follow-up inspection will occur. Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

    Observation: The cutting board in the pizza hut preparation area is chipping and in poor repair.

  • Plumbing / Maintained in Good Repair
    Observation: The following plumbing systems are not maintained in good repair:
    1. Handsink near the drive through with a small leak in the piping beneath the sink.
    2. Toilet in the men's room which is clogged and not properly functioning.

    Correction: Repair the plumbing fixtures.
  • Refuse / Outdoor Containers /Capacity
    Observation: The quantity of refuse exceeded the storage capacity of the exterior storage enclosure.
    Correction: Either replace the dumpster withone of larger capacity, obtain an additional dumpster, or increase the pick-up frequency of refuse so that bags of trash are not left on the ground of the refuse enclosure.
  • Physical Facilities Good Repair
    Observation: Observed a coving tile beneath the handsink near the walk-in refrigerator pulling off of the wall.
    Correction: Caulk the coving tile to the wall.
  • Ventilation Systems, Shall Be Clean
    Observation: The hood systems have not been cleaned within the past 6 months.
    Correction: Have the hood systems cleaned.
