  • Critical: Sanitize/Chemical/Manual & Mechanical/Concentration & Exposure Time
    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 4-501.114) The dish machine is not properly sanitizing dishes use the three compartment sink to sanitize.
    Correction: Cease use of the chemical sanitizing warewashing machine at this time. The warewashing machine shall not be used unless it is properly repaired and chemically sanitizing equipment, utensils, dish wares, glass wares, etc. at a concentration of at least 50-100ppm of chlorine for the required amount of time as specified by the warewashing machine's data plate.

    Observation: The handwashing station at the rear sushi area is being used for purposes other than washing hands. Clean food containers were improperly placed in the hand sink. Food container must be washed, rinsed and sanitized the stored in a clean dry area.
    Correction: A handwashing sink may not be used for purposes other than hand washing.

    Observation: Observed food employees using improper handwashing procedures. Employee failed to lather hands/arms with soap and scrub for at least 20 seconds. Employee washed hands for 20 seconds after being instructed by EHS.
    Correction: ALL food employees shall wash their hands in the following manner: 1) remove and discard used gloves, 2) turn on warm water at the hand sink, 3) rinse hands/arms with warm water, 4) lather hands/arms with soap and scrub for at least 20 seconds, 5) rinse hands/arms with warm water, and 6) dry hands/arms with paper towels and then turn off the water at the hand sink using the paper towel. Effective handwashing will aid in the reduction of foodborne illness and is an important part of the food employee training program.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils, or after engaging in any activity that could contaminate the hands. Employee failed to wash her hands after changing task. Employee washed her hands after being instructed by EHS.
    Correction: ALL food employees shall be washing their hands OFTEN especially after handling trash, using the rest-room, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction foodborne illness.
  • Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. Employee was drinking out of open to-go container. The employee's open drinking container was discarded during the inspection.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
  • Critical: Food Protection from Contamination / Separate Raw from RTE
    Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. Raw fish were improperly stored above avocados.
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc.
  • Dressing Areas & Lockers for Orderly Storage of Employee Clothing & Possessions
    Observation: Lockers or other suitable facilities are not provided for employees clothing and other possessions.
    Correction: Provide proper storage facilities for employees' personal belongings to prevent contamination of food, food equipment, and food contact surfaces.

    Observation: The floor in the walk-in refrigerator is not maintained in good repair.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

  • Critical: Certified Food Manager: Presence Required
    Observation: There is no Certified Food Manager (CFM) present in the establishment. The CFM permit is more than 30 days past the expiration date. Provide current CFM permit in 10 dates.
    Correction: It is unlawful to operate a food establishment unless it is under the immediate control of a CFM. It is highly suggested that you have more than one employee with the food manager's certification. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in enforcement action.

    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination.     Observation: The person in charge is not able to provide written procedures for the use of time as a public health control with the following potentially hazardous food item(s): sushi rice.

This visit was made to conduct a routine inspection. The following items need correction:
- Implement an Employee Health Policy that requires all employees to know the BIG 5 foodborne illnesses and all reportable symptoms and Certified Food Managers to know when to restrict or exclude sick employees

    Observation: Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: employees did not know BIG 5 foodborne illnesses or repoartable symptoms. This visit was made to conduct a risk assessment. Please note the following items:
1) Employee working must have their Northern Virginia Certified Food Managers Card. Please obtain your card and fax a copy to 703.746.4919 (addressed to me) within 2 weeks. Failure to do so may result in a civil penalty.
2) Review with new employees the employee health policy so that they are aware of the 5 illnesses and 5 symptoms required.
3) New employees need to be taught how to properly use time as a public health control on your sushi rice.
Note: Repeat violations may be issued a civil penalty of $50-$100 on future inspections.

  • Certified Food Manager: Certificate Process (repeated violation)
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card.
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be faxed to the Environmental Health Division at 703.746.4919. Failure to provide a copy of the photo identification card may result in further enforcement action.
