    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. CFM will use up this amount of chicken today and said he will have refrigerator adjusted cooler and not overload it. Unit temp. reading 36F before leaving.

    Observation: The following unit(s) was overstocked and is not sufficient in number or capacity to meet the food storage demands of the establishment. In Unit #1, there is sauteed chicken.
    Correction: Equipment for cooling and heating food, and holding cold and hot food shall operate properly and be sufficient in number and storage capacity to provide food temperatures as specified under Chapter 3. As discussed, the CFM will prepare less chicken at a time.
  • Toilet Tissue, Availability
    Observation: Observed that toilet tissue holder was not provided at each toilet. None have holder and all have a roll of toilet paper sitting on each toilet.
    Correction: Provide toilet tissue holder at each toilet to minimize hand contact with fecal waste.

The purpose of today's visit was to conduct a complaint investigation in conjunction with a routine inspection. The complainant alleged that the facility is dirty and old oil is used in the fryer. The dining area, floors, and counters were clean during today's visit. The fryer oil was changed today and is regularly changed. The complaint was not confirmed during today's inspection.

    Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. OBSERVED (1) RAW FISH OVER RAW SLICED ONIONS (2) RAW SHELL EGGS OVER SALSA VERDE (3) RAW MARINATED CHICKEN OVER RAW JALAPENOS IN THE WALK-IN-COOLER.
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc. REARRANGED
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: (1) 2DR SUPERIOR PREP COOLER (2) 2DR BEVERAGE AIR DISPLAY COOLER IN THE KITCHEN
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: (1) HOOD (2) SHELVING IN THE WALK-IN-COOLER.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required (corrected on site)
    Observation: (1) METAL PANS (2) PLASTIC PANS were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food. DISCUSSED WITH CFM.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room (repeated violation)
    Observation: There is no cover to the feminine napkin refuse container in the ladies room stall.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

The purpose of today's visit was to conduct a complaint investigation regarding concerns about temperature abuse of cooked foods, potential equipment and food cross-contamination issues. Did not observe any of the issues at the time of the inspection, but discussed with the Certified Food Manager about their cooling, cleaning, and food storage practices. Complaint was not confirmed at the time of the inspection.
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: Roast chicken in the chicken display case at 120F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. Time as public health used until chicken display case is repaired.
  • Physical Facilities Good Repair
    Observation: Observed that the tile floor in front of the oven has a hole and is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. Hole may be a safety hazard. Repair within 10 days and fax the invoice to the Health Department within 10 days.

The purpose of today's inspection is to conduct a routine inspection.
Dishwash: 3 vat sink
Discussed proper thawing methods with the Certified Food Manager:

  • Food Storage Containers, Identified with Common Name of Food (corrected on site)
    Observation: The following food items that are not easily identified by appearance were observed without a label: (1) RICE (2) MSG
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc. FOOD LABELED.
  • Thawing / Approved Methods (corrected on site) (repeated violation)
    Observation: Observed the following food thawing using an improper method: FISH THAWING IN A BUCKET OF WATER AT ROOM TEMPERATURE.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process. FISH THAWED UNDER RUNNING WATER.
  • Food Contact Surface/Smooth, Easily Cleanable
    Observation: Food contact surfaces of equipment and utensils are not finished to have a smooth, cleanable surface. OBSERVED FOIL LINING SHELVES IN THE SERVELINE AND SHELVES IN THE KITCHEN.
    Correction: Materials that are used in the construction of utensils and food contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be finished to have a smooth, easily cleanable surface.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board(s) along the (1) 2DR SUPERIOR COOLER (2) CUTTING BOARD IN KITCHEN are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Equipment / Non-food Contact / Visibly Clean
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: HOOD
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (repeated violation)
    Observation: Surfaces of ice bins and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
  • Microwave Oven Interior & Door Seals Cleaned Every 24 Hours
    Observation: The cavity of the microwave oven is observed soiled.
    Correction: The interiors of all microwave ovens including cavities, door seals, floors, walls, and ceilings shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure.

    Observation: Observed that the WALL AND CEILING BY THE BOILER is not maintained in good repair. HOLES IN THE WALL AND CEILING ARE COVERED WITH CARDBOARD. PAPER TOWEL DISPENSER IS IN DISREPAIR.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Observed that mops are improperly stored between use.
    Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.

    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: cooked rice and meat in the rice cooker (106F), white rice in hot well in customer serving area (110F).
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. Both were reheated and observe above 165F.

    Observation: Observed cutting boards located at customer serving area are not being cleaned at least every 4 hours.
    Correction: Food contact surfaces of equipment and utensils must be cleaned and sanitized at least every 4 hours to prevent contamination by bacterial growth.
