Today's visit is a follow-up to verify that the Norlake 2DR upright cooler has been repaired to hold at 41F or below, to verify that cold holding has remained corrected and to verify that the correct sanitizer level has remained corrected. EHS observed that the cooler and food inside were both holding below 41F and observed that the sanitizer concentration was correct. Facility is in compliance.
No violation noted during this evaluation.
02/09/2016Follow-up
The purpose of today's visit was to conduct a routine inspection. A follow-up inspection will be conducted within the next ten calendar days to verify that the Norlake 2DR upright cooler has been repaired to hold at 41F or below, to verify that cold holding has remained corrected and to verify that the correct sanitizer level has remained corrected. Additional active managerial control is needed in the areas of checking sanitizer levels and verifying proper equipment holding and food cold holding temperatures. Inspection time reflects time to obtain correction of cold holding.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) in the Norlake 2DR upright cooler were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    1) Three containers of sliced tomatoes all at 50F,
    2) Two containers of tuna salad at 49F and 48F,
    3) Seafood salad at 49F,
    4) Bags of shredded cheese mix at 51F,
    5) Egg patties at 48-50F (observed in the Duke 2DR cooler and had been recently moved from the Norlake 2DR upright cooler),
    6) Ten small chocolates at 50F.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. ALL ITEMS WERE IN THE UNIT OVERNIGHT AND WERE DISCARDED.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: ------------Norlake 2DR upright cooler observed holding at 51F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. EHS SPOKE TO OWNER VIA TELEPHONE AND PER OWNER A THIRD PARTY SERVICE TECHNICIAN WAS CALLED AND SCHEDULED TO ARRIVE WITHIN ONE HOUR.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site) (repeated violation)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the sanitize basin with a concentration of less than 150 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. CFM TROUBLESHOOTED AND PREPARED SANITIZER PER HAND SUCH THERE IS AN ISSUE WITH THE DISPENSING SYSTEM.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (corrected on site)
    Observation: Outside dumpster was observed uncovered on the right side.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
02/02/2016Routine
The purpose of today's visit was to conduct a routine inspection. EHS provided manager with and discussed the CFM guidelines. EHS observed some food items requiring date marking were accidentally dated as 1/12/15-1/14/15. EHS discussed with manager to remind employees to be careful to include the correct date to avoid confusion, the date on these items were corrected to 2/12/15-2/14/15. Overall the majority of food items requiring date marking were correctly dated and manager explained that corporate policy limits the refrigerated shelf life then less time then allowed in the Food Code. EHS observed food temperature logs noting food temperatures on the front line twice a day.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3-vat sink and solution for soaking wet wiping towels concentration of 50 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm based on the manufacturer's use directions and using the appropriate test kit.(Dispenser adjusted by CFM/Owner)

The Subway on Burke Centre Parkway near Kohl's is among the rudest staff of any subway in Northern Virginia. Consistently poor customer interaction. We have stopped patronizing this store because of their rude behavior towards customers, their very average food quality, and the uncleanliness of the store. Last time we were there they were counting cash before preparing our sandwiches; the sandwich maker refused to look at us, just barked at us, "Bread, Cheese." I just stared at him until he finally looked up with a sour expression. Unbelievable. There are too many places to get prepared food to put up with rudeness. The entire staff needs to be retrained by Subway, or be closed down.
