Today a ROUTINE INSPECTION of the facility was conducted. Thank you for accompanying me during this inspection as your participation allows me to assess areas at your facility that may require further clarification.
At this inspection Employee Health policy guidelines were discussed with staff present. Staff are aware of Employee Health and informed me that refresher training was conducted in December.

  • Critical: Certified Food Manager/Presence Required (corrected on site) (repeated violation)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.MANAGER PRESENT WAS UNABLE TO PROVIDE ME WITH A SERVE SAFE CARD/ORS CARD OR ANY PROOF OF HAVING PASSED A FAIRFAX COUNTY APROVED FOOD MANAGER EXAM
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. A CERTIFIED FOOD MANAGER WITH SERVE SAFE CERTIFICATE ARRIVED APPROXIMATELY 20 MINUTES AFTER START OF THE INSPECTION.
  • Certified Food Manager/Certificate Process (repeated violation)
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.THE CERTIFIED MANAGER WHO ARRIVED HAD SERVESAFE CERTIFICATE BUT NO FOOD MANAGER CARD FROM ORS INTERACTIVE.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action. PROVIDED CFM WITH HANDOUT OF INFORMATION ON OBTAINING A CERTIFIED FOOD MANAGER CARD.
  • Lighting Intensity/ Walk-ins & Dry Storage/ 10 foot candles
    Observation: Observed that inadequate lighting was provided in the walk-in refrigerators, walk-in freezers, and/or dry storage areas . LIGHT BULB IS NOT WORKING IN THE WALK IN FREEZER AND THERE IS NO LIGHT.
    Correction: Increase the lighting to provide a minimum of 10 foot candles at a distance of 30 inches off of the floor in these areas. DISCUSSED WITH MANAGER TO REPLACE THE BULB.

A ROUTINE INSPECTION of your facility was conducted today. There was no Certified Manager at the beginning of the inspection. Please note that it is ILLEGAL to operate the facility when it is not under the care of a Certified Food manager with valid card at all hours of operation. Manager has been notified that the Health Department may conduct a follow up inspection at a later date to verify that the facility is operating under the care of a certified manager at all times.

    Observation: There is no Certified Food Manager present at the beginning of the inspection. OBSERVED THAT THERE WAS NO MANAGER AT THE FACILITY, THE ONLY STAFF MEMBER PRESENT COULD NOT PROVIDE ANY PROOF OF HAVING SUCCESSFULLY PASSED /OBTAINED A VALID FOOD MANAGHER LICENSE.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam.   • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device:OBSERVED DICED TOMATOES AT 55F AND CUCUMBER DRESSING AT 57F IN THE 2 DR FLAT TOP REFRIGERATOR AT SERVICE LINE.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.CORRECTED: DRESSING WAS DISCARDED AND DICED TOMATOES MOVED TO THE WALK IN REFRIGERATOR.
  • Equipment / Good Repair / Operation (repeated violation)
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: OBSERVED THE DUKE 2 DR FLAT TOP REFRIGERATOR AT 60F USING A CALIBRATED THERMOMETER.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Dressing Rooms and Lockers to be Kept Orderly (corrected on site) (repeated violation)
    Observation: Observed employee clothing and other possessions stored in areas that may contaminate food, food-contact surfaces of equipment, single-service items, or single-use items.OBSERVED EMPLOYEE COAT, HAND BAGS AND SUCH PERSONAL POSSESSIONS STORED ON SAME SHELF USED TO STORE FOOD.
    Correction: Instruct employees to use the designated lockers and clothes hooks to prevent contamination of food, food equipment and utensils, and preparation surfaces.CORRECTED: DISCUSSED WITH CFM WHO REMOVED THE ITEMS FROM THE AREA.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the floor tiles and wall in the mop sink cupboard are dirty and in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the QUAT SANITIZER solution was measured at >500 ppm using a quaternary ammonium test strip This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of QUAT SANITIZER solution at 200-400 ppm. Verify concentration using the appropriate test kit.CORRECTED: ADJUSTED CONCENTRATION TO 400PPM AND DISCUSSED WITH CFM TO REGULARLY CHECK THE CONCENTRATION.

    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.Observed cartons of food stored on crates in the walk in freezer
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.CORRECTED: CFM REMOVED THE ITEMS AND STORED THEM ON THE SHELVES.

    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
