The purpose of this visit was to conduct a follow-up inspection on the Delfield 2dr cooler. Cooler air temp was observed to be at 40F, but according to the substitute air temp logs, the air temperatures were well over 41F. EHS was informed that the repairman stated that the cooler is having thermostat issues which caused the inconsistent air temperatures in the substitute temperature logs. Based on EHS recommendation, manager on duty has agreed to sell only non-dairy/non-potentially hazardous food items until the cooler is fully operational and consistent with the air temperature. A follow-up inspection will be conducted again on/or about April 6 (Wednesday). EHS observed official temperature logs to be inconsistent and inaccurate (which unit is what, etc.) EHS highly recommends to go over temperature log training. Observation: There is no Certified Food Manager present at the beginning of the inspection.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: milk (2x)- 50F and 47F, half&half 50F
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CFM removed all photentially hazardous items and the opened dairy wil be put on time as public health control (2hrs).

- Temp Meas Devices for Ambient Air Location
Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit: delfield 2dr cooler
Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.
- Handwashing Sink / Accessible at All Times (corrected on site)
Observation: The handwashing facility was blocked, preventing access by employees for easy handwashing.
Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use.

The purpose of today's visit was to conduct a pre-opening inspection. This facility is approved for operation and is hereby issued a Health Department permit. Until your official permit is received in the mail, this report shall serve as your permit. Correct all violations listed within 30 days.

Observation: The exposed wood ledges by the ice machine and register stand are not easily cleanable.
Correction: Ensure these surfaces are easily cleanable either by re-covering them or painting them to create a seal.

Observation: The CM had a certificate demonstrating successful completion of CFM exam but not photo ID card. .
Correction: Obtain a CFM photo ID card from ORS Interactive within 30 days.
