A ROUTINE INSPECTION of your facility was conducted today. Please ensure staff have Food manager card from ORS interactive (information provided). Discussed with manager that staff need to monitor milk bottles kept outside refrigeration units and replace it back into cooling units immediately after use. QUESTIONS: Please call 703-246-2444
- Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: OBSERVED VANILLA SOY MILK AT 48F AND WHOLE MILK AT 63 IN THE 1 DR DELFIELD FLAT TOP, SOME OF THESE ITEMS WERE BOTTLES WEREOBSERVED FROM START TO BEING PLACED ON COUNTER TOP
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MILK WAS DISCARDED , DISCUSSED WITH MANAGER THAT MILK BOTTLES AND CARTONS HAVE TO BE PLACED BACK INTO REFRIGERATON UNITS IMMEDIATELY AFTER USE.
- Certified Food Manager/Certificate Process (repeated violation)
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. MANAGER HAS PROMETRIC CERTIFICATE.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.OBTAIN FOOD MANAGER CARD FROM ORS INTERACTIVE IN 10 DAYS AND FAX ME A COPY OF THE CARD.

Observation: Observed that paper towels were not provided at each handwashing sink. OBSERVED NO PAPER TOWELS IN THE HAND WASH SINK AT THE FRONT AND THE BACK.
Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.DISCUSSED WITH MANAGER , PAPER TOWELS WERE PROVIDED.

Observation: The SINK STOPPERS of the 3 VAT SINK WASH AND SANITIZE COMPARTMENTS are MISSING AND BOTTLE CAPS ARE BEING USED INSTEAD.
Correction: Equipment components such as stoppers, doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. OBTAIN STOPPERS FOR YOUR 3 VAT SINK.

Observation: The establishment has not posted the permit to operate in a conspicuous location. OBSERVED PERMIT WAS POSTED ON A BOARD INSIDE THE BACK OFFICE
Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue. CORRECTED: CFM MOVED THE LOCATION OF THE PERMIT TO THE FRONT .
- Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
Observation: Water from the handwashing sink in the back kitchen areas, front service area and restrooms was measured at a temperature less than 100°F.OBSERVED TEMPERATURE IN THESE SINKS WAS BETWEEN 76F AND 78F.MANAGER REPORTED THAT THERE WAS A PROBLEM IN THE MIXING VALVE OF THE WATER HEATER AND A WORK ORDER HAS BEEN PLACED FOR REPAIRS.
Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
- Physical Facilities Good Repair
Observation: Observed that there was a leak with water dripping from the ceiling tile in the back area near the hand wash sink.
Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
