    Observation: The commercially processed vegetables and meat in the refrigeration unit was not properly dated for disposition after opening. EHS observed no date marking of opened foods in the walk in coolers.
    Correction: Mark a "consume by" date on commercially processed RTE foods at the time the original container is opened. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The person in charge failed to state the minimum cook temperatures and times for potentially hazardous food. EHS asked PIC the cook temperature of chicken and he answered 145. Educated PIC to cook to a minimum of 165. Left handout with proper temperature.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.

Asked PIC about employee health and symptoms of disease and he could not answer any of my questions correctly. Left PIC a handout in chinese so that he would have all necessary information.

    Observation: Unwrapped or uncovered food in the walk in and reach in refrigerator. EHS observed uncovered food throughout restaurant. EHS observed these foods and others uncovered: Broccoli and cabbage uncovered on counter, Onions, egg rolls broccoli, cabbage etc. uncovered in walk in refrigerator, and beef uncovered in walk in freezer. Employees covered at my request.
    Correction: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
  • Utensils - In-Use - Between-Use Storage (repeated violation)
    Observation: Dispensing utensils improperly stored between uses. EHS observed scoop handle down inside flour. PIC corrected.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Wiping cloths improperly stored between use. EHS observed damp wiping cloths left out on food prep shelves. PIC corrected
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Sweet and sour sauce hot holding at improper temperatures. EHS observed sweet and sour sauce hot holding at 70 degrees. Educated PIC about hot holding. PIC reheated to 170 degrees.
    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.

    Observation: Chicken cold holding at improper temperatures, EHS observed chicken cold holding at 52 degrees.PIC discarded.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: The nonfood contact surface of the fryers, storage shelves and reach in refrigerators has accumulations of grime and debris. EHS observed grease and dirt build up around fryers, shelves and reach in refrigerators.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Correction: Store equipment and utensils covered or inverted to prevent contamination while in storage.

    Observation: prep sink faucet in poor repair. EHS observed leak at prep sink faucet.
    Correction: Repair and maintain all plumbing components ans fixtures.

    Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment. EHS observed kids bicycle stored in kitchen.
    Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.

    Observation: Many meat products stored in refrigeration units in bags not food safe containers. Some foods stored in aluminum cans bought in. These type containers have chemicals in them that breakdown in the food and can never be used fro storage of food.
    Correction: Protect food from miscellaneous sources of contamination.

    Observation: Grease from the exhaust hood system at the cook line was observed dripping onto food contact surfaces below. Observed dripping and drip marks on hood system right above cook line. Vents need cleaned right away to prevent a fire and contamination of foods.
    Correction: Alter or replace the hood system to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, single-service, and single-use articles.
  • Equipment - Cooking and Baking Equipment (repeated violation)
    Observation: The cavity of the microwave oven is observed soiled and the deep fryers need cleaning. Ensure equipment are maintained clean. Establish a daily cleaning schedule for all equipment.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.

Facility did a really good job of correcting violations. Cleaning has improved, almost all items observed date marked, Cold holding units were with in normal limits, food handler cards are up to date, dispensing utensils are correct, hair restraints are in place and items off the floor. Prepared sauces in walk in still need label with common name and date marked for disposition

PIC stated there is no health policy in place.
    Correction: Have the license holder require employees or applicants report to the person in charge any information about their health and activities if they are diagnosed with or had a past illness from Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus. Provided facility with brochures on illness and educated PIC on signs and symptoms and when to not work with food.
