    Observation: A food employee failed to wash his or her hands during food preparation when changing tasks. Meat cook was handling raw chicken breasts with gloved hands and then cooked foods without changing gloves/washing hands.

    Correction: Provide an outer opening screen with a mesh that is 16 to 1 inch. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: A supply of toilet tissue is not available at each toilet.
    Correction: A supply of toilet tissue shall be available at each toilet. To minimize hand contact with fecal waste, toilet tissue is necessary for hygienic cleaning following use of toilet facilities. Toilet tissue must be supplied to meet the demand.

    Observation: The premises is not free of items that are unnecessary to the operation or maintenance of the establishment. Old equipment was stored behind the establishment.

The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely.     Observation: The chlorine chemical sanitizer used in the sanitizing solution does not meet the criteria specified under section 7-204.11, is not used in accordance with the EPA-approved manufacturer's label use instructions, or does not have a minimum temperature based on the concentration and pH of the solution. Containers were being washed in 3 part sink without use of sanitize step.
    Correction: A chlorine chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at exposure times specified under paragraph 4-703.11(C) shall meet the criteria specified under section 7-204.11 Sanitizers, Criteria, shall be used in accordance with the EPA-approved manufacturer's label use instructions, and shall have a minimum temperature based on the concentration and pH of the solution as listed in the following chart

    Observation: The person in charge did not know the 5 big foodborne illnesses that are reportable to the regulatory authority. The certified food manager was informed and given a handout.
    Correction: The person in charge shall notify the regulatory authority when a food employee is diagnosed with an illness due to: (a) Norovirus, (b) Hepatitis A virus, (c) Shigella spp., (d) Enterohemorrhagic or Shiga toxin-producing Escherichia coli, or (e) Salmonella Typhi.

    Correction: 1. Sour cream(46ºF) and cheese(48ºF) in the Kelvinator upright refrigerator. 2. Sour cream(47ºF) in the True 1 door upright refrigerator. The temperature was adjusted on the refrigerator units.
  • Temperature Measuring Devices
    Observation: The "True" 3 door counter refrigerator outside and the "Frigidaire" upright freezer are not equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
    Correction: Cold or hot holding equipment used for potentially hazardous food (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display. This section does not apply to equipment for which the placement of a temperature measuring device is not a practical means for measuring the ambient air surrounding the food because of the design, type, and use of the equipment, such as calrod units, heat lamps, cold plates, bainmaries, steamtables, insulated food transport containers, and salad bars. A permanent temperature measuring device is required in any unit storing potentially hazardous (time/temperature control for safety) food because of the potential growth of pathogenic microorganisms should the temperature of the unit exceed Code requirements. In order to facilitate routine monitoring of the unit, the device must be clearly visible.

    Observation: The "True" upright refrigerator and the "Kelvinator" upright refrigerator are not maintaining foods at 41ºF or less.
    Correction: Equipment shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 [Materials for Construction and Repair] and 4-2 [Design and Construction]. Proper maintenance of equipment to manufacturer specifications helps ensure that it will continue to operate as designed. Failure to properly maintain equipment could lead to violations of the associated requirements of the Code that place the health of the consumer at risk. For example, refrigeration units in disrepair may no longer be capable of properly cooling or holding potentially hazardous (time/temperature control for safety) foods at safe temperatures.

    Observation: The "True" 3 door countertop refrigerator was observed stored outside and next to a stack of cardboard boxes for recycle.
    Correction: Cleaned and sanitized equipment, utensils, laundered linens, and single-service and single-use articles may not be stored: (1) In locker rooms
  • Liquid Wastes and Rainwater/Nonsewage
    Observation: Condensate drainage and/or other nonsewage liquids including rainwater are not drained and disposed of according to law. Water from the air conditioning unit was observed draining into a bucket adjacent to the bathrooms.
    Correction: Condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Liquid food wastes and rainwater can provide a source of bacterial contamination and support populations of pests. Proper storage and disposal of wastes and drainage of rainwater eliminate these conditions.
  • Refuse/Storage Areas, Rooms, and Receptacles, Capacity and Availability
    Observation: There is no receptacle provided outside the establishment where recyclables or returnables are placed.
    Correction: A receptacle shall be provided in each area of the food establishment or premises where refuse is generated or commonly discarded, or where recyclables or returnables are placed. Garbage containers should be available wherever garbage is generated to aid in the proper disposal of refuse.
  • Physical Facilities/Repairing (repeated violation)
    Observation: The ceiling tiles in the dry storage area are water damaged and a few floor tiles are not maintained in good repair.
    Correction: Physical facilities shall be maintained in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. This requirement is intended to ensure that the physical facilities are properly maintained in order serve their intended purpose.

    Observation: Flies were observed in the kitchen.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by eliminating harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Effective measures must be taken to control their presence in food establishments.
  • Critical: Sanitizers, Criteria/Chemicals for Food Contact Surfaces (corrected on site)
    Observation: The chlorine sanitizer in the wiping cloth bucket was observed at 200 PPM ++ and it does not meet requirements. The sanitizer was diluted.
    Correction: Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940 Tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (food-contact surface sanitizing solutions). Chemical sanitizers are included with poisonous or toxic materials because they may be toxic if not used in accordance with requirements listed in the Code of Federal Regulations (CFR). Large concentrations of sanitizer in excess of the CFR requirements can be harmful because residues of the materials remain. The CFR reference that is provided lists concentrations of sanitizers that are considered safe.
