The purpose of today's visit was to conduct a risk factor assessment inspection. EHS discussed with CFM proper thawing, cold holding, time as a public health control and fully stocking all handwashing sinks.
  • Critical: Cold Holding of Raw Shell Eggs at Ambient Temperature of 45°F or less (corrected on site)
    Observation: Observed raw shell eggs stored at room temperature.
    Correction: Shell eggs that have not been treated to destroy Salmonella shall be stored in refrigerated equipment that maintains an ambient temperature of 45°F or less. DISCARDED.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site)
    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.

The purpose of today's visit was to conduct a routine inspection. CFM on duty was knowledgeable. The following good active managerial control practices were observed today: organized and easily accessible documentation for parasite destruction for eight fish species and correct documentation of time as public health where used. EHS provided CFM with parasite destruction freezing log (as future reference). EHS recommended that raw shell eggs used for the cooking stations be kept in a Cooler holding 41F or below until needed or time as a public health control needs to be used.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows:-----scoop for rice stored in still/stagnant room temperature water.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. MOVED TO DISH MACHINE TO BE WASHED AND SANITIZED.

    Observation: The following fish offered for service or sale in the ready-to-eat form was not frozen to ensure parasite destruction:--------flounder (fluke).
    Correction: Before service or sale in ready-to-eat form, raw or partially cooked fish shall be: 1) frozen and stored at a temperature of -4°F or below for a minimum of 7 days in a freezer, or 2) frozen at -31°F or below until solid and stored at -31°F or below for a minimum of 15 hours, or 3) frozen at -31°F or below until solid and stored at -4°F or below for a minimum of 24 hours.     Observation: Manufacturer containers previously for seaweed wrappers were observed reused for the storage of sauce and vegetables.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.

    Observation: Observed the handsink in the warewashing area is being used to hold a metal pot.

Today's visit was to investigate a complaint received by the Health Department on January 26, 2015. The complainant left a voice message on January 22, 2015 at 5:37 pm reporting a roach infestation. The EHS discussed complaint with CFM upon arrival. CFM was not aware of any related complaints during the last few weeks. Facility receives pest control service weekly. EHS observed and reviewed services invoices from 1/6/15, 1/13/15, 1/20/15 & 1/27/15. EHS inspected bar area, customer grill areas and kitchen and did not observe any evidence of cockroaches or other pest during the inspection. Complaint cannot be confirmed

    Observation: Harborage conditions exist. food debris accumulate under the hibachi food prep stations in the dining areas. Food debris was noted along the base of the hibachi cooking table walls.

Eliminate harborage conditions to control and prevent the presence of pests on the premises. Thoroughly clean under the these cooking stations to properly remove the food debris eliminating one of the food sources for pests. You may use long reaching brooms/brushes and mops to clean these areas.

At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:

    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Several bottles of cleaning chemicals were observed stored on a food shelf and above cleaned linens and utensils in the hallway to the employees restrooms.
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination. (chemicals bottles were relocated).

This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.

    Observation: The following food items that are not easily identified by appearance were observed without a label: spices, flour, clear bulk soups.
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.

    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.

    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor in the walk-in freezer..
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

    Observation: The person in charge (PIC) who arrived during the inspection, has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
