Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Carmelized onion cold holding at improper temperatures.
Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Cooler observed malfunctioning and all TCS foods were appropriately, voluntarily discarded or relocated during the inspection. Temperature line check procedures reviewed and observed to be effective.
Physical Facilities - Cleaning Frequency and Restrictions (corrected on site) Observation: Dishmachine located in the bar area buttons and area around the door were observed to be in need of cleaning.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Discontinue use of bar dishmachine until drain is unclogged.
Temperature Measuring Devices Observation: There was no temperature measuring device located in the front drawer coolers or the burger patty cooler.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature. Correct in 15 days.
Critical: Backflow Prevention Device, When Required* (corrected on site) (repeated violation) Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin. Hose with nozzle attached to mop sink.
Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch. Suggest connecting a backflow device to the faucet so the hose may be left attached.

Discussed employee health policy with PIC. New cleaning procedure is being established for ice machine. Wash/rinse/sanitize utensils in contact with PHF's every 4 hours.
