The purpose of this visit was to conduct a risk factor assessment. Chlorine sanitizer final rinse dish machine: 100 ppm. Quaternary ammonium sanitizer in wiping cloth bucket: 300 ppm. Provided copy of date marking fact sheet and CFM guidelines to PIC.
- Critical: Certified Food Manager/Presence Required (corrected on site)
Observation: There is no Certified Food Manager present at the beginning of the inspection.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. NOTE: PIC with required credentials arrived during the inspection.

The purpose of this visit was to follow-up on control measures for cockroaches. PIC provided a service invoice from a licensed pest control operator for inspection/treatment dated 8/18/15. PIC advised inspections/treatments have been scheduled for two times per week. No cockroach activity was observed during the inspection.

Observation: Replace the damaged gaskets and broken door hinge on the 3 dr prep cooler.
Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

The purpose of this visit was to conduct a routine inspection. Chlorine concentration dishmachine: 50 ppm. Quaternary ammonium sanitizer in 3 vat sink: 300 ppm. A follow-up inspection will be conducted tomorrow. An invoice from a licensed pest control company, dated either 8/18 or 19/ 2015 must be provided at the time of the inspection.
- Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced tomato/ salsa/ chicken wings/beef in 3 dr prep cooler.
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. NOTE: PIC voluntarily disposed of all TCS food that was out of temperature in the cooler.
- Equipment / Good Repair / Operation
Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 3 door prep cooler.
Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. NOTE: Cooler was not functioning at the start of the inspection. PIC adjusted plug in electrical socket to return cooler to service.

Observation: Observed that the electrical plug box serving the 3 door prep cooler is not funtioning satisfactorily to provide a reliable flow of current to the cooler.
Correction: Have the electrical plug box repaired/replaced by a licensed electrician.
- Critical: Controlling Pests / Methods to Use When Pests Are Found
Observation: Methods being used are not adequately controlling the presence of cockroaches within the facility.
Correction: Cockroaches are capable of transmitting disease to people by contaminating food and food-contact surfaces. If cockroaches are found on the premises methods shall be used to control their presence. NOTE: Have facility inspected/treated by a licensed pest control within 24 hours and as often as necessary thereafter.
