The purpose of today's visit was to conduct a risk factor assessment inspection. Please update and add a complete consumer advisory disclosure and reminder statement as discussed to the dine-in menus, carry-out menus and daily specials menu at the next menu printings. Hookah is offered for the outdoor dining area. Please increase active managerial control in the area of cooling in the short term and for the long term. EHS provided owner and manager with handouts on cooling in English and Spanish.
  • Critical: PHF(TCS) Cooled from 135°F to 70°F within 2 Hours (corrected on site)
    Observation: The following cooked potentially hazardous food(s) are not being adequately cooled from 135°F to 70°F within two hours:----------partially cooked wings at 62-75F after cooling 2 - 2.5 hours in the Walkin Cooler.
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within two hours to prevent the growth of harmful bacteria. Foods that have been temperature abused are to be discarded. SPLIT ONTO SEVERAL METAL TRAYS AND PLACED INTO FREEZER TO RAPIDLY CHILL TO 70F OR BELOW.

The purpose of today's visit was to conduct a risk factor assessment inspection. Please update and add a complete consumer advisory statement as discussed to the dine-in menus, carry-out menus and daily specials menu at the next menu printings. Please refer back to the handouts provided at the May 26, 2015 routine inspection. Please fax or email a third party service invoice for the True 2DR pizza prep cooler by Friday, December 25, 2015. If invoice is not received by requested deadline a follow-up inspection will be conducted on December 28, 2015 to verify that the unit is able to maintain at 41F or below. Hookah is offered for the outdoor dining area.

    Observation: Employees are not aware of or are not practicing proper methods to rapidly cool potentially hazardous food (PHF).
    Correction: The Person in Charge or certified food manager shall be monitoring the methods used to cool down large batches of hot foods. The foods shall be cooled rapidly using adequate methods and approved temperatures and times to prevent growth of vegetative bacteria, toxin formation, and/or germination of spores.

    2) Garlic in oil at 90F sitting out at room temperature.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Please correct the Thursday evening menu regarding the oysters offered raw within ten calendar days and fax or email an updated menu no later than Friday, June 5, 2015. Please update and add a complete consumer advisory statement as discussed to the dine-in menus, carry-out menus and daily specials menu at the next menu printings

    Observation: A review of the daily specials menu and thursday evening menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked:-------raw oysters.----------Also observed that a complete consumer advisory disclosure statement is missing on the main dine-in menu and carry-out menu for all skirt steak menu items, grilled bbq ribs, rib-eye and ahi tuna.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.

    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.

    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: Several cooked or prepared foods inside the walk-in refrigerator (eg: chicken broth, beef broth, mac & cheese, sauteed vegetables) were noted date marked consistently as required.
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine.

    Observation: The food-contact surfaces of the following equipment were not observed sanitized due to improper operation of the warewashing machine.
    Correction: All utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning. After the ware washing machine is fixed clean all utensils again at the machine to ensure that they are properly sanitized before use.

A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
