03/23/2016Routine
The purpose of this visit was to conduct a complaint investigation in conjunction with a risk factor assessment inspection. Complainant allegedly bought coleslaw from facility and stated it was spoiled. PIC was not aware of the complaint and no other complaints were known to the facility. There has been no recalls or damaged product and equipment. PIC went over the coleslaw process from delivery to the customer. Coleslaw comes precut and prepared in bags with the dressing in a separate bag. Facility washes the coleslaw, lets it air dry and then mixes in the dressing (contains egg). After mixing the product, the coleslaw is portioned out into cups and then stored in the walkin cooler and the Beverage Air 1dr cooler by the drive-thru. Maximum time to keep coleslaw is 2 days. EHS did not observe any damaged product and equipment was in working condition. Based on today's observations, the complaint can not be confirmed at this time

The purpose of this visit was to conduct a routine inspection. Person in charge reviewed HACCP plan with EHS during today's visit. The purpose of today's visit was to conduct a risk factor assessment inspection. EHS observed facility clean and organized. EHS recommended to leave hot foods, cooling down in Walkin Cooler uncovered and containers unstacked in order to allow rapid cooling. EHS provided manager with and discussed cooling sign and methods in English and Spanish.

The purpose of today's visit is to conduct a routine inspection.
Fix the mop sink drain within 48 hours.
Facility Data:
Hood: Every 6 months and filters every week.
Grease trap: Outside cleaned every month.
Pest Control: Twice a month.
REMINDER: Monitor cooling methods and time.

  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: Open top coolers for raw chicken. (the one attached to unit is broken).
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The door gaskets of the following unit is damaged: Beverage Air 1DR undercounter near cashier.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Plumbing / Maintained in Good Repair
    Observation: The sink basin at the mop sink is slow to drain.
    Correction: A plumbing system shall be maintained in good repair.
  • Physical Facilities Good Repair
    Observation: Observed that the floor tiles around the 3-vat sink area is not maintained in good repair and accumulates water in between tiles.
    Correction: Re-grout the floor tiles..

    Observation: A food employee was observed eating, smoking, or drinking in an area other than designated break area while preparing/handling food or contacting clean food contact surfaces. Observed one employee eating and one employee drinking in food prep area.
    Correction: Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. Employees shall not eat, drink, or use tobacco in the kitchen areas especially while preparing food for customers.

  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: The handsink across walk-in refrigerator needs new caulking.
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The door gaskets of the following units are damaged: 3DR flat top refrigerator and 1DR undercounter refg front and one dipper well handle broken.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Equip/Utensils/Linens/Single-Service/Invert, Cover (corrected on site)
    Observation: Clean single service chicken boxes were observed stored with the food-contact surface facing upward.
    Correction: Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered/inverted.
