    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Walk in cooler: Chicken patty 44F, Yogurt 45F, Cream cheese 45F, corn/bean salad 44F
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MANAGER DISCARDED PRODUCTS.

The equipment placement changes have been noted from the previous inspection. Repair all items noted in the report and provide a copy of the service report.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site)
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. MANAGER CLEANED ICE BIN.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the floor by the serving lines, wall in the dry storage room, and ceiling in the dry storage room is not maintained in good repair.
    1. Holes noted in the floor on both the right and left side of the last piece of equipment on the serving line.
    2. A wall in the dry storage room is cracked and in need of repair.
    3. There are missing ceiling tiles in the dry storage room. Replace tiles.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

Manager has removed food from the serving line, cleaned the wall containing the ants, and called maintenance to determine the next course of action to eliminate pests.

    Observation: The following food item(s) is observed to be unsound or adulterated: Observed live ants crawling in and on the fruit in the individual plates.
    Correction: ALL food shall be safe, unadulterated and honestly presented as specified under 3-601.12. FOOD WAS DISCARDED.
  • Physical Facilities Good Repair
    Observation: Observed that the tiles in the dry storage room by the window are not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
