Sanitizing Solutions, Testing Devices Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a Chlorine (for dishwasher) test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
Warewashing - Mechanical Warewashing Equipment, Wash Solution Temperature Observation: The wash temperature of the low temperature chemical sanitizing ware wash machine is not meeting the manufacturer's specifications. The minimum temperature is 120ºF and the actual temperature is 90 degrees F.
Correction: Discontinue the use of the low temperature chemical sanitizing ware wash machine until it is working properly. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.
Plumbing System Maintained in Good Repair Observation: Plumbing connections at the backflow preventer on the dishmachine is leaking.
Correction: Plumbing systems and components shall be maintained in good repair.
Physical Facilities - Cleaning Frequency and Restrictions Observation: Floor under the fryer noted in need of cleaning.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Routine
Generally, facility observed to be clean and well ordered. Food temperatures observed were within required range. Automatic dishwashing machine chlorine sanitizer at proper level of 100ppm and in bucket for wet wiping cloths also at 100ppm. Operator to send plan for corrective action to health dept. by COB on 1/7/15 to email: elizabetha.campbell@vdh.virginia.gov
Critical: Handwashing Lavatory* Observation: There are not an adequate number of handwash lavatories on site. (See 12 VAC5-421-2280) There is no handsink in rear prep area at pizza prep station.
Correction: Install additional lavatories to increase the employee's opportunity to wash hands routinely. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies.
Floors, Walls, and Ceilings - Cleanability Observation: Floor or floor covering in walk in cooler is absorbant and is not smooth and easily cleanable.
Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.
Physical Facilities - Cleaning Frequency and Restrictions Observation: Wall juncture at rear of 3-compartment sink is moldy and noted in need of cleaning.
