The purpose of this visit was a follow-up to the inspection conducted 3/2/16. PIC presented recent invoice from licensed PCO dated 3/7/16. No evidence of recent pest activity was observed.
  • Physical Facilities Good Repair
    Observation: Observed that the roof is leaking in the area of the 3 dr pizza prep cooler.
    Correction: Repair the roof as necessary to stop the roof leak. Poor repair and maintenance compromises the functionality of the physical facilities.

The purpose of this visit was to conduct a routine inspection in response to a complaint investigation. Dishmachine activated thermolabel. A follow-up inspection will be conducted on or about 3/11/16.

    Observation: The following food items were found hot holding at improper temperatures using a calibrated food temperature measuring device: MELTED BUTTER ON COOKTOP AT 115F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. MOVED TO STEAMER.

    Observation: The following food item WAS observed cold holding at improper temperatures using a calibrated food temperature measuring device: COOKED SAUSAGE IN 2DR PREP COOLER AT 64F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MOVED TO ANOTHER COOLER.

    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests. OBSERVED 2 CANS OF RESIDENTIAL PESTICIDE.
    Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination. DISCARDED
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the BLEACH solution was measured at GREATER THAN 200 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of BLEACH solution at 50-200 ppm. Verify concentration using the appropriate test kit. CORRECTED CONCENTRATION TO 100PPM.

At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:

    Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by providing an air gap as specified under 5-202.13.

11/21/2013Routine
The purpose of this visit was to conduct a routine inspection. Water heater: State SBF1004000NFASME Dish machine: HOBART AM-14. EHS provided employee health policy (EHP) in English and Spanish, and discussed EHP procedure in detail with PIC. PIC to provide documentation from fish supplier that smoked salmon is treated for parasite destruction. PIC also to provide documentation that TRUE 2 door reach in has been satisfactorily restored to fully functional condition. Documentation to be provided within 48 hours. PIC will suspend use of True unit for cold holding of PFH foods until it is restored to fully functional condition. Provided parasite destruction handout.

    Correction: If raw or partially cooked fish are served or sold in ready-to-eat form, the person in charge shall record the freezing temperature and time to which the fish are subjected and shall retain the records for the frozen fish at the food establishment for 90 calendar days beyond the time of service or sale to a consumer. If the fish are frozen by a supplier, a letter the fish supplier stipulating that the fish is frozen as required shall be provided. PIC will provide records from supplier that salmon is treated for parasite destruction.

    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: 2 dr prep unit at cookline.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
