    Observation: Damaged tomato sauce cans stored in a location that does not segregate the product.
    Correction: Damaged, spoiled, or recalled food shall be held in the food establishment as stated under 6-404.11. Do NOT store damaged, spoiled, or recalled food with food that is going to be used for the consumer. DISCARDED 2 CANS AND TRAINED STAFF TO SEGREGATE DAMAGED CANS.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: PIZZA PREP COOLER LEFT SIDE- HAM AT 44F, SAUSAGE AT 44F, CHICKEN AT 44F.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. REDUCED THE AMOUNT OF FOOD IN BINS AND REMOVED MEASURING CUPS FROM THE BINS TO ALLOW THE TOP TO CLOSE. NEW COOLER IS ON ORDER.
  • Refuse Areas / Enclosures / Receptacles / Good Repair
    Observation: The waste storage container outside has a broken lid.
    Correction: Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair. REPLACE THE BROKEN LID.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site)
    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests. OBSERVED 2 CANS OF RESIDENTIAL PESTICIDE.
    Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination. DISCARDED CANS.

  • Critical: Package Integrity & No Dented Cans/ Specifications for Receiving (corrected on site)
    Observation: The following food item(s) from damaged packaging were found offered for sale or service: several dented cans of pizza sauce
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit. CFM removed cans and labeled it as damaged to return to vendor
  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

    Observation: Water from the handwashing sink by the side door and bathroom handsinks were measured at a temperature less than 100°F and 3-vat sink was less than 110F.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
  • Handwashing Sink / Accessible at All Times
    Observation: The handwashing facility located by the side door is blocked, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
  • Handwashing Sink / Used for Hand Wash Only
    Observation: Observed the handsink by the pizza line had food debris (tomato seeds and juice).
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Separation of Toxics by Spacing or Partitioning (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. spray bottle of glue by pizza boxes. removed.
    Correction: All poisonous and toxic materials shall be stored to prevent contamination of food, equipment, utensils, linens, and single service items by separating the poisonous and toxic materials by spacing or partitioning.
